2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development

Subject: Overlook Estates Sub #2, Lot 11 – Eagle River Rd
Septic system design
Dear On-Site Services Engineer:
The owner of the above lot intends to build a 3-bedroom home on the property, so we are
submitting this permit application for the construction of a new well and septic system. The
attached site plan identifies the location of the home as well as the proposed well, septic location
and alternate site. No conflicts exist between this proposed system and any other well or septic
system, whether on this lot or adjacent lots.
The ground surface on the lot is moderately sloped with grades between 10-20% running to the
south. There is one small section of existing slope that is at or under 35% where we are proposing
a new septic system on the south side of the property. The current design meets all steep slope
provisions as stated in 15.65.210.B.4 of the MOA Wastewater Code. An existing subdivision test
hole was used for this design and we intend to confirm soils with another test hole prior to
construction. Historical soils tests for this lot and the surrounding lots indicate consistent GP/GM
soils with percolation rates under 1 minute per inch, and no groundwater in the month of October.
Contour lines are shown on the site plan showing the grade and direction of flow. Stormwater
drainage will not impact this septic system. The new trench will be constructed parallel to the slope
as much as possible.
The new system will be greater than 100’ from all wells and surface water. The well will be located
greater than 50’ from the existing stream near the west propertyline per requirements of Title 21
Chapter 7. The alternate site is located greater than 6’ away from the primary site, and the tank
will be a minimum of 5’ from proposed trenches as well as 10’ from the building foundation.
Please refer to the attached plan, profile page, and test hole log for the septic design. If this design
is followed, there will be no adverse impacts to adjacent properties.

Conditional COSA request
Dear On-Site Services Engineer:
The developer of the above lot has completed the home and septic system, and is finalizing the
transfer of the title to the new owners. The septic system is installed and was inspected during the
installation. However, during the preparation of the final engineered record drawings, we
discovered some minor deficiencies that need to be corrected before the septic permit can be closed
out.
Our final measurements showed that the monitoring tubes had been knocked around during
backfill, and do not reflect the 6” effective depth of the trench. The laterals were also disturbed
and are approximately 3” out of level along their length, in opposite directions. Finally, the cover
over the septic tank is short by approximately 6”.
When we informed the contractor, we discovered that he had left Alaska and was spending time
with family in Oklahoma. The developer attempted to hire another contractor to fix the trenches,
but the weather turned and the ground has since frozen. The system is currently working, and there
is no threat of contamination or system failure over the winter, but it would be extremely difficult
to dig up the lateral and fix the pipes with the ground frozen.
We request that a conditional COSA be issued for this system until the spring, when the ground
thaws and the system can be excavated and fixed. We have received three bids from contractors
and placed 150% of the highest bid in escrow for this purpose.
