Dear Mark Cebuhar:
An On-Site WaterAVastewater Permit, number SW010444, issued by this office for a single-family
system, will expire on October 18, 2002. This permit was valid for 365 calendar days.
If this was a well permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as-built inspection report must be sent to
this office for review, approval and documentation. This as-built' inspection report must be signed by
the licensed Professional Engineer who inspected the installation of the system. As-built inspection
reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A new permit for a second year may
be issued for a fee of $100.00 if the reneWal application is received on or before the expiration date of
the original permit.
When applying for a new.permit, the fees are: $400.00 for a wastewater permit and $150.00 for a well
permit.

Reft Septic Tank Upgrade for Lot 5A, Sandi Heights Subdivision.
To whom it may concern:
The existing 4 bedroom house is currently served by an on-site septic system and community
well. The conventional septic system contained two septic tanks, a 1000 gallon tank, and a 500
gallon tank. The 1000 gallon tank has collapsed. We are proposing to remove the exisitng tanks
and replace them with a new 1250 gallon septic tank.
I am unaware of any adverse impacts this installation would have on adjacent wells or septic
systems. If you have any questions, please contact us at 337-6179. Thank you for your

NOTE: Attached is a site plan drmving, a design drawing,, and a 5 page construction
specification letter which are all part of the design package for this septic system.

The Required Size of the Soil Absorption System Is:

The length dimension is the length(in feet) of the trench or drainfield. The
depth of a trench or pit is the distance between the surface of the ground and
the bottom of the excavation(in feet). There is no set width for trenches.
The gravel depth is the minimum depth of gravel between the outfall pipe and
the bottom of the excavation(in feet).

Permit applicant has the responsibility to inform this department during the
installation inspections of any wells adjacent to this property and the number
of residences that the well will serve.

Backfilling of any system without final inspection and approval by this department
will be subject to prosecution.
Minimum distance between a well and any on-site sewage disposal system is 100 feet
for a private well or 150 to 200 feet from a public well depending upon the type
of public well. Minimum distance from a private well to a private sewer line
is 25 feet and to a community sewer line is 75 feet. Well logs are required
and must be returned to this department within 30 days of the well completion°
Other requirements may apply. Specifications and construction diagrams are
available to insure proper installation.
