SITE NARRATIVE
Lot 12 A Sandi Heights Subdivision is a fully developed lot with a single family home in a residential neighbor
hood. The structure is of wood frame construction, and remaining lot area is landscaped with lawn. The lot is
~urrounded on all sides with homes of similar construction and age.
All of the lots in the area are connected to a community wilt so there are no individual wells in the surrounding
area. The water service for this lot enters from the street to the front of the house.
The area is generally flat with a gentle slope to the south east. This lot slopes up slightly from the street to the
front of the house and slopes down from the back of the house to the rear lot line at 5-8%. There are no streams
or active drainage features in the vicinity.
An existing system was found on file at DHHS. It consists of a 1000 gal septic tank draining into a wood crib
cesspool with a 12'x12' drain field. The surrounding lots all have similar systems

THE FINAL INSPECTION. INVERTS AND SWING TIE LOCATES SHALL BE TO THE NEAREST TENTH OF A FOOT. THESE

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of afl wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed abqve. Any reliance upon or use of this report by any

AWWU is currently not using this utitity easement and has no objection to the encroachment.
