Per email correspondence with the MOA onsite department (attached), inspections were called in with the MOA on 8/5/94 and 8/13/94, however, no record drawings are in the MOA records. Per my conversations with MOA onsite employee Rebecca Carroll, P.E., due to there being record of inspections being called, the following assumptions could be made: • The system was not installed too deep and meets MOA separation distances to groundwater • The drainfields were sized as designed and permitted • The lateral lines and/or spacing meets MOA code requirements • Materials used meets MOA specifications (sand, rock, lateral line material etc....) A septic adequacy test was performed by GEG on 11/22/2019 and found the drainfield meets MOA absorption requirements for a 2 -bedroom home (see attached conditional COSA). It should be noted that the east drainfield was filled to 100% and did not drop during a 3 hour recover period. Per the request of the owner, the existing septic tank is to be replaced with a conditional COSA addressing several deficiencies associated with the system. They are listed as follows: • The flow diverter must be repaired and/or replaced • The east drainfield does not have adequate soil cover. The contractor must confirm insulation over the drainfield. If it does not have insulation, insulation and/or soil cover must be added • The mound does not have 3:1 slope. Soil cover must be added to meet MOA mound slope requirements. • Various pipes are broken and need to be cut so that a watertight cap may be placed over the vent. • The existing drainfield encroaches on surface waters to the south; we request that your department issue a variance form drainfield to surface waters to the south from 100 feet down to 85 feet. The area between the existing bed and surface waters is heavily vegetated. In addition, this encroachment has likely existed for the life of the system. In addition, during a site visit on 11/22/2019 it was determined that the outlet of the tank is lower than the east drainfield. This was verified on 12/12/2019. It was also determined that the foundation cleanout invert is lower than the existing drainfields. In short, a gravity flow septic tank could not be installed without replacing the east drainfield at a lower elevation. Given that the design states the drainfield would be installed at grade, it seems reasonable to assume that if the drainfield were lowered, it would not meet MOA code required separation to groundwater. Therefore, we are proposing to decommission the existing septic tank and install a new 1500 -gallon EPDXY coated steel STEP tank. In addition to the design, the record drawing for the profile of the bed is attached. Once the new tank is installed, the record drawings for the STEP tank will be submitted. We are unaware Mf any adverse impacts this installation and/or waivers would have on adjacent wells or septic systems. If yop hp4e any questions, please contact us at 337-6179. In February, 1994 the well was drilled to a depth of 170 FT. Please reference the attached Well Log. During the time since the permit was issued, Mr. Svendsen has slightly revised the desired location of the bed. Additionally, per the On -Site Services Section's Engineer Bulletin 94-3 of June 1, 1994, we would like to modify the design to narrow the bed width. On October 24, 1993 we performed all additional percolation test/soils log (test hole #3) to the east of the previous test holes. Test hole #3 had a percolation rate of 9 min/inch, and after monitoring for eight months, the highest groundwater level observed was 7 FT. Because the other test holes had groundwater at 4.5 FT, we are proposing a similar mound, however it will be split in order to keep the width less than 15 FT. Note that we are proposing to place the eastern- most bed over the location of test hole 93. This is due to a desire to maintain a 20 FT separation from the house in the event that a foundation drain is installed around the perimeter of the house. Because the test hole was relatively shallow (and will have had nearly one year to settle before construction), we feel that any significant settlement which might occur has already taken place. With a percolation rate of 9 min/inch, the application rate is .5 GPD/SF. Based oil three bedrooms, the daily design flow is 450 gallons/day. Two test holes were excavated at locations indicated on the attached site plan. Percolation rates were 8.7 min/inch and 11.1 min/inch for Test Holes #1 and 42 respectively. Water was encountered at 4.5 FT both during the testing, and at the end of the ground water monitoring period. Thus a mound system will be required to maintain the 4 FT minimum vertical separation distance between the bottom of the drainfield and the water table. Based on four bedrooms, the daily design flow is 3 X 150, or 450 gallons/day. The application rate (for both test holes) will be .5 Gal/Day/SF. The required area is thus 450/.5, or 900 SF. We have selected an area (45 FT X 20 FT) for the mound down -gradient from the proposed house. The bottom of the drainfield (bottom of the washed rock) will be at the existing grade. Prior to placing the drainfield, the approx. 6"-8" organic layer will be removed, and brought to original grade with a leveling course of clean sand.
