Septic System Permit Request
Design Narrative
This is a design narrative for a permit to install a septic system to be issued for the garage on this
property. The proposed system will serve an, existing garage with a future 1BR ADU. Currently the lot is
developed, and the existing septic system is connected to the primary residence. The proposed system
will utilize a new 1000g septic tank and conventional drain field. This lot and the surrounding developed
lots are served by private wells. There are no surrounding wells within 100' of the proposed septic
system.
1. Soils. One test hole was performed in the vicinity of this system by PES in June 2017, and groundwater
was monitored for at least seven days. Ground water was observed at a depth of 4.5' below the
surface in the test hole monitor tube in June of 2017. Bedrock was not encountered in the test hole to
10.0'. Based on the results of the percolation tests and overall soils appearance; an application rate of
0.45 gallons/day/square foot was used for a conventional wastewater system, in the area of the test
hole.
2. Soil Absorption System Design.
a. See Sheet 1 of the design package.
3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain field.
The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage
ditches, including the mapped creek on the next lot to the south.
4. Topography: The existing topography on the lot generally slopes from north to south, with a
slope of approximately 20% in the area of the drain field. There are no steep slopes within 50' of the
proposed drain field. The proposed drain field will maintain 50' separation all steep slopes.
5. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification that
the review is complete and that there are no further comments is received from MoA On-Site
Department, the note will be removed and "Issued for Construction" drawings will be issued.
The proposed installation will not affect the future development of this or the surrounding lots.

SPECIAL PROVISIONS TO SPECIFICATIONS
1. ALL CONSTRUCTION SHALL BE INSTALLED AS SPECIFIED IN THE MOST CURRENT EDITION OF THE MUNICIPALITY OF
ANCHORAGE STANDARD SPECIFICATIONS (MASS) FOR COMPONENT PARTS AND MATERIALS USED IN CONSTRUCTION OF
ON-SITE WASTEWATER DISPOSAL SYSTEMS AND AND IN ACCORDANCE WITH AMC 15.65 AND 15.55.
2. SCOPE OF WORK: INSTALL NEW TANK AND DRAIN FIELD.
3. GROUNDWATER WAS ENCOUNTERED AT A DEPTH OF 4.5 FEET BELOW EXISTING GRADE AS EVIDENCED BY THE SOIL
TEST HOLE. IF AN APPARENT WATER TABLE IS OBSERVED IN ANY OF THE EXCAVATIONS LESS THAN 4.5 FEET BELOW
EXISTING GRADE NOTIFY THE ENGINEER IMMEDIATELY.
4. THE CONTRACTOR (BOTH WELL AND SEPTIC SYSTEM CONTRACTORS) SHALL HAVE ANY WELL LOCATION AND SEPTIC
LOCATION STAKED AND ANY LOT LINE AND WELL RADIUS SHOWN ON THE PLAN WITHIN 30 FEET OF THE PROPOSED
SYSTEM STAKED BY A REGISTERED LAND SURVEYOR BEFORE STARTING THE WORK.
5. THE CONTRACTOR IS RESPONSIBLE FOR ALL R.O.W. AND OTHER REQUIRED PERMITS, OTHER THAN THE ATTACHED.
6. THE CONTRACTOR SHALL CALL FOR LOCATING OF ALL BURIED UTILITIES.
7. THE CONTRACTOR SHALL PROVIDE 24 HOUR NOTICE TO THE ENGINEER PRIOR TO START OF WORK. ALL SURVEYING
AND LOCATES SHALL BE IN PLACE PRIOR TO NOTIFYING THE ENGINEER.
8. THE CONTRACTOR SHALL NOTIFY THE ENGINEER OF ANY DISCREPANCY BETWEEN THE APPROVED DRAWINGS AND SI i L

10. THE CONTRACTOR SHALL PROVIDE PHOTOGRAPHS OF THE SYSTEM INSTALLATION TO INCLUDE BOTTOM OF
EXCAVATION, TOP OF PIPE WITH CLEAN-OUTS AND MONITOR TUBES INSTALLED, INSTALLED TANK AND FINAL GRADING.
11. THE CONTRACTOR PROVIDED DATA (UPON WHICH THIS RECORD DRAWING IS BASED) APPEARS TO REPRESENT THE
PROJECT AS CONSTRUCTED. 1HIS DATA IN CONJUNCTION WITH THE PERIODIC FIELD OBSERVATIONS BY THE

Dear Greg Ondeck:
An On-Site Water/Wastewater Permit, number SW010299, issued by this office for a single-family
system, ~vill expire on August 3, 2002. This permit was valid for 365 calendar days.
If this was a well permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as-built inspection report must be sent to
this office for review, approval and documentation. This as-built inspection report must be signed by
the licensed Professional Engineer who inspected the installation of the system. As-built inspection
reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or ~vastewater
disposal system, a new permit must be obtained from this office. A new permit for a second year may
be issued for a fee of $100.00 if the renewal application is received on or before the expiration date of
the original permit.
When applying for a new permit, the fees are: $400.00 for a wastewater permit and $150.00 for a well
permit.

As requested we are resubmitting our design for a septic leachfield raising the drainfield level to a total
depth of 2.5 feet. We are also requesting a well drilling permit as the current well production does not
meet the owner's requirements for well floxv. The new well site is as shown on the site plan.

Dear Mr. Cross:
The proposed septic upgrade/well and septic system will have very limited impact on adjacent properties
for the following reasons:
1. The surrounding lots are large and have adequate area for septic and well placement.
2. Immediate neighboring septic systems are all +30' distance.
3. Reserve space is adequate, due to absorption capacity and well location.
4. Drainage will not be affected and is not a major consideration in our design.
This is the 2na septic system for this lot. The original was installed in the early 70's and while we cannot
find any documentation, a test hole was excavated and the system inspected as per the owner. We have
excavated to uncover the existing tank. This is a single compartment concrete tank in usable condition. It
i's proposed that a second tank be installed in series, incorporating the lift station in this tank and therefore
providing a second compartment and the lift capacity ..... :: .....

SPECIFICATIONS FOR ON-SITE SEPTIC SYSTEM
LEGAL: Lot 20, Sunny Valley
August 2, 2000
A. GENERAL
1. The well and septic plan are for a single family residence only.
2. The drawing and or site plan shall be a part &this specification.
3. All materials and workmanship shall meet the Anchorage Department of Health and State
Department &Environmental Conservation requirements.
4. All soil tests are advisory to the design and are to be verified or modified in the field by the engineer.
5. Ali excavations and depths are advisory and ar~ to be verified in the field by the contractor to meet
Municipality of Anchorage, Department of Environmental Conservation requirements.
6. It is the responsibility of the owner to obtain all necessary permits or easements and to locate any
adjacent multi-family wells.
7. The excavation is to be exactly in the area shown on the site plan, any deviation requires engineer
approval.
B. SEPTIC TANK
I. Existing tank is to be utilized; a new 500-gallon Orenco systems tank is to be installed after the
existing tank, wiring to be to Anchorage code by licensed electrician.
C. DRAINFIELD
1. The drainfield is to be installed to follow the existing ground contours.
2. The trench bottom elevation is to be adjusted to meet gravel depth specifications. The bottom of the
trench shall be level, plus or minus 1.5".
3. The total depth of the trench excavation is not to exceed 2.5' at any point referencing the average
ground level at the drainfield location .......
4. The effluent line within the new trench shall be laid level within 0.03'.
5. The trench gravel is to be covered with typar fabric material.
6. Mounded soil or combination of soil and extruded board insulation to a depth oft or equivalent is to
be placed over the leaehfield.
7. The area over the trench is to be finish graded to prevent ponding &surface water runoff.
8. The septic tank and leachfield must not be closer than 100' to any existing private well, 150' to any
Class "C", or 200 feet to any larger community well.
RECOMMENDED LEACHFIELD DIMENSIONS:
TOTAL DEPTH = 2.5' GRAVEL DEPTH = 1' under pipe, 2" over pipe
TRENCH LENGTH = 66' TRENCH WIDTH = 5'
SOIL RATING = 1.2 GPD/ft2 BEDROOM CAPACITY = 3
SEPTIC TANK = 1000 existing + 500 gallon Lift Station
Twenty-four (24) hours notice required for all inspections.
