In 2004 the above property was developed with a single-family home, and Douglas T. Kenley
applied for a well and septic permit. Inspections were called in on the 5t' and 6" of April, 2004 but
we were unable to verify the installer. The muni sent a letter to the property owner reminding them
that the permit had expired, but nothing came of it.
After receiving a notice of violation from the muni this year, Forge Engineering was called to
perform what inspections were possible and close out this permit. On July 25, 2019, our inspectors
came on-site to take measurements of the distances and elevations of the septic tank and field. We
did not expose the tank or do any excavation to verify materials or standards used in the
construction of the system- we were only able to determine the layout of the system. Original grade
was determined to be of similar elevation to the original ground we found near the drain field
during our site visit.
We found 2.8 feet of cover over the tank (1.2 feet shy of the minimum requirement), and were
unable to verify if insulation was used. However, the owner stated that after living in the home for
the past 16 years, they have never had an issue with freezing or anything indicating that there may
be a problem with the operation of the septic system. This indicates that the tank may be insulated.
In any case the system has not had a problem with freezing. Additionally, there is a small portion
of one side of the tank that has pavement over it, according to the survey and plan as -built. Due to
the location of the tank and the driveway layout, there would be very limited movement of motor
vehicles, if any at all, on this portion of the driveway, and it is not directly over the tank. Therefore,
we are able to determine that these minor discrepancies will have no adverse impact on this septic
tank and field on this property.
We have put together the attached record drawings and inspection report based on our
measurements and observations.

PERCOLATION TEST RESULTS
AND
GENERAL SITE INVESTIGATION REPORT
This letter is a request for a well and septic permit on the above -referenced property.
On April 5, 2004, an inspection was conducted on the site, consisting of approximately 50,250
square feet, which is located on McCrary Road near the intersection of Rainwater Circle. The
inspection revealed that the property was average treed, with spruce being predominate and some
birch and poplar present. The leach field will be placed to follow the contour lines of the lot.
Two perc tests were performed on April 7, 2004, (see attached soil logs). Both test holes were dug
to a depth of 16'. It is proposed to use a 3' wide x 6' E.D. x 85' long trench, and a new 1250 gallon
tank will be installed for the future 4 -bedroom house.
S R. S Engineering had done a soil log for each of the eight lots in the subdivision some time in the
1990s as part of a comprehensive survey. Their test hole on Lot 4 was also dug to a depth of 16'.
We have not been able to obtain their percolation rate.
On-site observation and physical survey show that there are no water wells or private wastewater
disposal systems within a 100' radius of the proposed system. The proposed system will have no
measurable impact on surface or sub -surface drainage, or on drainage from adjacent lots. It appears
that there is no potential for contamination of adjacent water wells or streams from known sources.
Also, the proposed development will not prohibit future development of any neighboring lots.
