Ref: Proposed Well and Septic System for Lot 2, Goodman Subdivision
To whom it may concern:
The proposed 5 bedroom house will be served by a private well and septic system. Three test
holes were dug on the property. The drainfield will be designed around the 30 foot radius of test
holes #2. We are proposing that a 1500 gallon septic tank and also dual deep trench type
drainfields be installed. Comments regarding the design are summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring,
and the percolation test results. It is our opinion that due to the overall appearance of the soils, an
application rate of 0.6 gallons/day/1t2 should be used.
2. TRENCH DESIGN:
a. Percolation Rate: 21.8 minutes/inch
b. Proposed Application Rate: 0.6 gallons/day/ft
c. Number of Bedrooms: 5
d. Design Flow: 750 gallons per day
e. Minimum Absorption Area: 1250 f12
f. Total Depth: 10 feet (max.)
g. Effective Depth: 6 feet
h. Width: 2.5 feet
i. Reduction Factor: N/A
j. Minimum Length: 55 feet long each (I 10 feet long total)
k. Effective absorption area = 1320 ft2
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic
system. In short, there are no slope concerns.

4. TOPOGRAPHY: As can be seen on the attached design drawing, the average topography in
the area of the proposed drainfield is a 1-5 percent slope running approximately northeast to
southwest. In short, there are no slope concerns.
I am unaware of any adverse impacts this installation would have on adjacent wells or septic

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of On -She Systems
Approval (COSA) based only upon the representations given In paragraph 4 by an Independent professional civil
engineer registered in the State of Alaska. Certificates of Onsite Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of Onsite Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water

Certificate of On -Site Systems Approval # 060574
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Lot#Lof Goodman
Subdivision. This inspection revealed an arsenic concentration of 22.8
micrograms per liter (ug/L) for the property's well water sample. The
Environmental Protection Agency (EPA) has established a maximum
contaminant level (MCL) of 10.0 ug/L for public drinking water systems.
While private wells are not subject to this regulation, EPA standards are
based on existing health information and can therefore be used to gauge the
relative quality of water from private wells. Information on arsenic is
available from the On -Site Water and Wastewater Program website
(www.muni.org/Onsite) or at 343-7904.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

During a recent adequacy test on the septic system for Lot 2 of Goodman
subdivision, 67 and 70 inches of standing water was observed in the
absorption fields. This indicates that approximately 95% of the absorption
area is inundated. Although this system passed the adequacy test, the
remaining life expectancy may be limited.
This advisory must be attached to all copies of the subject Health Authority
Approval.
