After review of the well decommissioning procedure performed by Bailey's Backhoe on an out ofservico well at Lot 1 of Winter Subdivision, the Municipality of Anchorage (MOA) On-Site Water & Wastewater Program (On-Site) has determined that the procedure is not in accordanco with the code. Shown below are the items that do not appear to be in compliance, and the requirements to satisfy them: 1. The well casing was filled with "dirt", while the code requires the casing to be filled with bentonite. Ref. AMC 15.55.060 L.I.e. To Satisfy:. Using company letterhead; provide a signed original letter with a detailed description of the "dirt" used to fill the well and where it came from. 2. Bailey's Backhoe has not been certified by On-Site as a well driller or well pump installer. Ref. AMC 15.55.060 L. .To Satisf~ Cease and desist from the decommissioning of any other single family wells within the MOA. Note: future well decommissioning attempted by Bailey's Backhoe within the MOA may result in the issuance ora citation, possibly incurring fines and/or the loss of the company's MOA certification to do septic system excavation. AK 99503 Phone: (907)-74-5257 January 6, 2009 Municipality of Anchorage Development Services Department On-Site Water & Wastewater Program 4700 South Bragaw Street Anchorage, AK 99519-6650 Re: Septic System As-built - Lot I Winter Subdivision MOA Permit Number SW080236 To Whom It May Concern: The installed on-site septic system was generally constructed as depicted on the original design that was submitted with the October 29, 2008 permit application. The location of the septic tank was slightly modified to account for the service line coming from a more central location of the house, in turn improving the accessibility for the equipment used. Please reference the attached on-site septic as-built drawings with the final plan and profile information for the constructed system. As requested in the MOA permit, a percolation test was performed in the reserve area (see attachment). Because of the construction activity at the time of the percolation test no soil logs could be completed. Because construction of the bed had begun an additional percolation test was not performed within the absorption area. Despite neither of these tests having been performed I believe there is adequate historic information that quantifies this as a reliable on-site septic system with ample area for a reserve bed of similar size. Primarily all soils discovered in the excavation and construction of the entire septic system (tank, piping, and absorption) appeared very similar in visual characteristics to that found in the original test hole. The groundwater also was verified at a similar depth of around 8 feet, or greater, throughout. This area is relatively flat and has similar vegetation to surrounding properties. In reference to MOA records we have compared the adjacent properties; Lot I of Betty W. Subdivision to the east and Lot 2 of Winter Subdivision to the south. On November 10, 1988 Eagle River Engineering Services monitored groundwater 8 feet below grade on Lot I of Betty W. Subdivision. Their soil log references poorly-graded sand mixes and compares similarly with the materials we found in our test hole. Referencing an October 2002 S&S Engineering soils log/percolation test collected in the southeast corner of LOt 2 of Winter Subdivision shows groundwater being discovered at a depth of 8 feet and monitored up to 6'-2" with silty-sands and gravels. In addition, a soil log and percolation test was performed on Lot I of Winter Subdivision by NorthRim Engineering on May 23, 2006. These tests show poorly-graded gravels with an initial groundwater depth of 11 feet with a depth of 9 feet after monitoring. 801 W. Fireweed Lane #201 I of 2 1365 E. Decommissioning of one well was required by the permit if both wells were not being used. Per a verbal statement from Bailey's Backhoe the northern most well was decommissioned. We are still awaiting a written verification on means and methods used to complete this task, and will forward this documentation to the MOA upon receiving. Robert Bell and Associates was retained by Sieve Orr Construction to provide third party inspection servicas on the installation of the septic system on Lot I of Winter Subdivision. Three inspections were performed: The first inspection took place on November 5t~, 2008. The primary draln~eld excavation was completed and ready for washed rock placement. The second inspection took place on November '/th, 2008. All washed rock had been placed and piping was installed from tile t,anlc to the end of the trench. Washed rock was placed 2 to 4 inches above the lop of the piplng. Geofabric was placed on top of washed rock ready for backfill. All wastewater piping and the septic tank was insulated with 2" of direct bury bored foam as frost protection, in addition to the 3 feet of earth cover. All monitoring tubes and clcanouts were installed and prepared for backfill. A third inspection was performed on November 14e, 2008. The septic tank and llne connecting to the house were installed. Compaction and bedding had been performed, but not backfilled. Clennout~ for the llne from the build~ng to the septic tank had been installed. A final inspection was performed on November 1'/e, 2008. The site was graded and the cnnlractor was demobilized from the site. Through all inspections, we found th~ construction to be in compliance with the plans and specifications as approved by the MOA for this project. Workmanship and quality of construction were more than adequate. Box 196650 Anchorage, AK 99519-6650 Re: Well and Septic System Design Lot 1 Winter Subdivision To Whom It May Concern: Site Summary: A proposed 4-bedroom house is being constructed on the 0.91 acre parcel descried as Lot I of Winter Subdivision. The property is located in the Peters Creek area at the southeast corner of the Valley Avenue and Bowery Lane intersection. The topography of the site is relatively fiat with a majority of the property gently sloping towards the northwest. One neighboring well was found approximately 85 feet north of the northeast property corner. All other existing wells are greater than 100 feet from the proposed system. Domestic Waste Water Calculations & Septic Design: Our firm inspected a test hole which was excavated in the general area of the proposed infiltration area. Below the top two feet of topsoil we found coarse sands and gravels (SP-GP) down to a depth of 4 feet. From the depth range of 4 to 6 feet we found medium sand with some silt (SP-SM). Medium and coarse sands (SP) were found in the depth range from 6 to 11 feet. Groundwater was found at a depth of 8.5 feet below grade. The test hole was excavated to a total depth of 11 feet. The field inspector also performed a percolation test which resulted in a percolation rate of 5.5 minutes per inch. Using M.O.A's wastewater application rate table and the observed percolation rate, an application rate of 0.7 gpd/s.f, was used. With the logged soil characteristics and depth of groundwater we recommend using a bed for the · absorption system. Using 2 inches of 40 psi direct bury board foam insulation (Dow Styrofoam Extruded Polystyrene Insulation or equal) and an additional 2 feet of fill over the entire system allows 4 feet of effective cover to be maintained and ensures 4 feet of separation from the groundwater table. (interpolated from MOA Title 15.65, Table 1) Required Absorption Area = 600 gpd + 0.7 gpd/s.f. = 858 s.f. Length of 15 ft. Wide Bed = 858 s.f. + 15 ft. = 58 ft. Construction or New Septic System: A new septic tank will be installed and connected to the proposed service line from the building. A bed system will be connected at a central manifold. The bcd will have three distribution lines spaced 5 feet apart and 2.5 feet from excavation walls. Distribution lines will be placed on top of at least 6 inches of filtering rock (3/4" to 1-1/2" washed rock) used for the bed. Monitoring pipes will be installed at each comer of the bed, and cleanouts will be installed at the end of each distribution line. The bed will be filled with additional washed rock to ensure 2 inches cover is met above the lines, and will be covered with a permeable geotextile fabric to prevent fines from infiltrating and damaging the absorption system. Two inches of 40 psi direct bury board foam insulation (Dow Styrofoam Extruded Polystyrene Insulation or equal) shall be placed over the entire system with 3.3 feet of backfill to obtain a minimum 4 feet of effective ground cover. Cleanouts shall be installed on the service lines going in and out of the septic tank. The septic tank and tight line piping installed shall be on undisturbed native soils or in trenches that have been adequately compacted. Maintenance of Septlc System: Routine maintenance shall be performed on the system, including pumping of the septic tank at least every two years. Using garbage disposals and flushing of non-degradable items (coffee grounds, cigarette butts, tampons, paper towels, etc.) should be avoided. The owner should limit heavy surface loads on the area to maintain the structural integrity of the system. It is approximately three feet above grade and marked with several red horizontal stripes. It is capped and locked. This monitoring tube existed when we began our site investigations in September 2008. Its origin, function, and depth am unknown, and were not directly referenced in the on-site wastewater design prepared by F. Robert Bell and Associates.
