Prior to work that we had done on the well (hydrofrac), Sullivan Water Wells had installed a pump and found that the production at that time was .12 gpm. Troy Davis homes subsequently requested that we frac the well, which we did. After frac testing of the well our findings were as follows: August 4, 2014 - 11:30pm the test pump was started and pumped continually (cycling on and off electronically when the well ran dry) until 10:30am the following morning. The flowmeter reading at 11:30pm on 8/4/14 was 890360, the flowmeter reading on 8/5/14 at 10:30am was 894250. A brief test was then commenced to conclude the overall findings. The test was ended at 10:48am with a flowmeter reading of 894307. Our findings are, on the last cycle, that the well pumped 39 gallons in 11 minutes or 3.56 gpm. The derated estimate of the yield of the well at the time of our testing is: 2.8 gpm. Please bear in mind, that 3,638 gallons were pumped from the well prior to our conclusion. The remaining trenches vary from 6.8' to 8'. The bedrock level at the westerly location was found at 13' below the surface. The trenches were placed lower in the ground and no fill material crossed the lot line. Soil conditions at this location are composed of clean gravel with sand and silt with percolation rates under 5 minutes per inch. Very little migration of effluent is anticipated. We therefore request a waiver be issued allowing the trenches to be 4' from the lot line. Sincerely, Michael E. Anderson, P.E. ANDERSON ENGINEERING P.O. We are therefore requesting a permit be issued for the construction of a septic system to accommodate the home. A well is currently in place on the lot. The attached Site Plan and backup documentation identify the location and configuration of the existing well and proposed septic system along with the alternate site. Also identified on the plans are the existing wells and septic systems on surrounding lots. No conflicts exist between the proposed well and septic system and wells or septic systems on adjacent lots. Drainage arrows are shown on the site plan showing the grade and direction of flow on the lot. The drainage pattern will not be changed as a result of this project. The test holes placed on the lot in the area of the absorption trench and alternate site indicate the presence of silty sand with some gravel (SM). The percolation rate of the material was determined to be 1.5 to 4.8 minutes per inch. Groundwater was found in Test Hole No. 3 at 11.5' but the other test holes were dry. Bedrock was encountered at 14' in Test Hole No. 1, 4' in Test Hole No. 2 and at 13' in Test Hole No. 3. Test Holes will be placed during construction to verify the depth of bedrock. The location of the verification test holes is shown on the site plan. Filter sand will be required to elevate the absorption system above ground to achieve the required 6' separation from bedrock. An application rate of 1.0 gallons per day per square foot was used in the design of the absorption system. We are proposing to place a new 1,500 -gallon STEP tank and 5 new trenches totaling 120' long by 5' wide by .5' effective to treat and dispose of the effluent generated in the house. The elevation of the distribution line in the easterly absorption trenches will be 2.5' above the existing ground surface. Organic material will be removed to 1' below ground. Coarse sand will be placed to raise the absorption trench above ground. The elevation of the absorption trenches will vary depending on the actual depth of verified bedrock encountered. A determination will be made in the field after consultation with the City as to the final elevation of each trench. A minimum of 3' of fill or 2" of insulation and 2' of fill will be placed over the absorption trenches and 4' over the septic tank. The difference in elevation of the distribution line will require the placement of adjustment valves at each trench. The valves will be used to insure the same fluid Lot 2, 011ie Walker Subdivision No. 2 May 31, 2014 Page Two pressure in the distribution line. The valves will be adjusted during the flow test and the pressure measured. The results will be recorded and submitted with the certified as built. The existing septic system has not been located on the property and no documentation exists to assist with the location. The system will be decommissioned in accordance with Municipal Code when it is found on the property. 30'00paa ftr•A The ground surface on the lot in the area of the septic system slopes to the east at a shallow grade for approximately then drops dramatically. A waiver is attached to allow the location of the furthest east trench to be less than 50' from a break in grade greater than 25%. The new trenches will be constructed parallel with the slope as much as possible in conformance with Municipal requirements. The new septic system will be constructed a minimum of 100' from all wells and any surface water and 10' the proposed and existing water services in the area. If the system is constructed in accordance with our design the following statements apply: The system, if constructed as designed, will have no adverse impact on the wells in the area or those to be constructed in the future. 2. The system, if constructed as designed, will have no adverse impact on existing septic systems in the area or those to be constructed in the future. 3. The system, if constructed as designed, will have no adverse impact on reserve space, either surface or subsurface, on any lots located in the area. 4. The system, if constructed as designed, will have no adverse impact on drainage patterns in the area. The current drainage pattern will be maintained. Sincerely, Michael E. Anderson, P.E.
