November 411, 2019 Municipality of Anchorage Development Service Department On -Site Water & Wastewater Program 4700 Elmore Road Anchorage, AK 99507 Ref: Septic System Installation Creekview; Lot 4 To whom it may concern: During GEG inspections of the subject installation, there were several items that did not meet MOA onsite code. They are as follows: 1. A pre -construction layout did not take place with the contractor and Garness Engineering Group, Ltd. 2. The testhole monitoring tube pipe was damaged during the foundation excavation and non-functional for its purpose. It did not extend to the required depth and therefore could not be used for groundwater monitoring 3. The new trench does not run parallel with the slope contours. 4. Upon our arrival the trench was over excavated (approximately 12.92 feet deep) on the uphill side. 5. Most of the trench was installed outside the 30 -foot radius of T.H.#1 6. The trench appeared to be in the surface water setback (100' setback was not flagged). Per conversations with the owner on 5/29/2019, a surveyor went onsite after inspection and confirmed that the trench was in the surface water setbacks. The contractor then decommissioned (reportedly by removing the drainrock) and "moved" the trench outset the 100' setback without GEG concurrence or inspections. 7. Per the owner, the abandoned portion of the trench (which encroached on surface waters) was backfilled with native material. The excavated drainrock was then used in the "new portion" of the trench. It is unknown how "clean" the drainrock was prior to being "reused". Also, it is unknown how the excavation and backfilling of the decommissioned portion effected the remaining portion of the drainfield. 8. Based upon item #4 and item #5, a new testhole was excavated to confirm the separation distance to groundwater. See the attached soil log with groundwater monitoring. Please see the attached soil log for T.H.#2 which shows soil classifications and groundwater monitoring. Also, see attached profile drawing which shows the trench profile and separation distance to groundwater. In addition, Garness Engineering employee David Garness, P.E. met with Deb Wokenfuss at the MOA onsite department regarding the percolation test for the trench portion that extends outside the testhole radius. Per MOA records, the percolation rate for Creekview Estates; Lot 3 (T.H.#2) was 17min/inch (to the east of our client's lot). Per MOA records, the lot to the west Creekview Estates; Lot 5, the soils percolated at <1 min/inch. In short, it is reasonable to believe that the soil around the trench has a similar percolation rate to our initial testhole. It should be noted that MOA onsite employee Tim Ecklund was onsite for the initial bottom of trench inspection and agreed with GEG assessment of the soil classification/s. In short, based upon these facts, an additional percolation test was not performed, and it is reasonable to believe the drainfield is not undersized for a 3 -bedroom home. Groundwater Monitoring: Testhole (TH#2) was excavated to a depth of 20.5 feet to confirm separation distance to groundwater/impermeable soils. The trench was excavated approximately 12.92' below the existing grade (including fill above original grade (OG)). As stated previously, the initial trench location was not encompassed by the 30' radius (prior to being moved by the owner and contractor). After the seven days groundwater was 16.92' below grade at T.H42. Elevation shots were taken at the testhole and the groundwater elevation was found to be 81.8 at the testhole. It should be noted that natural topography was sloping towards the drainfield. In theory, groundwater would be lower in elevation at the drainfield. Per conversations with the owner, he would like to construct a 3-bedroom home and septic system. We are proposing to install a new 1000-gallon HDPE septic tank and deep trench type drainfield. One testhole was excavated on the property and the drainfield is designed around its 30-foot radius. Comments regarding the design are summarized as follows: 1. SOILS: See the attached 2018 soil log which shows soil classification, percolation test, and groundwater monitoring. As can be seen on the attached soil log there is a GP material that perked faster than 1 min/inch from 0.5 feet to 5.5-6.0 feet. We are proposing that a sand filter not be utilized since there is a GM to SM material that perked at 10.4 minutes/inch below that layer. In short, the underlying soil profile will prevent effluent from entering the groundwater table at an accelerated rate. Additionally, the proposed trench will span both soil layers with approximately 4.5-5 feet of the effective depth being in the GM to SM layer and 2.5-2 feet in the GP layer. It should also be noted that as the soil was being excavated, it was "harder digging" with depth. We are proposing an average application rating of 0.7 gpd/ft2. Comments regarding the design are as follows: 2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications. 3. SURFACE WATERS: There is a creek to the south, however, it was professionally surveyed in and the proposed drainfield is greater than 100 feet from its location. 4. TOPOGRAPHY: See attached 100 scale with topography information. We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems. Box 196650, Anchorage, Ak 99519-6650 (907) 343-7904 Attention: Dan Roth Ref: Exploratory Well Permits for a new subdivision to be known as Creek View Estates (subdivision of Tract B, Rexview Terrace S/D) Mr. Roth: We have been h/red by Mr. Jeff Regnart, the developer of this subdivision, to obtain exploratory well permits and perform an aquifer study for the purpose subdividing the referenced property. Attached is a site map that shows the majority of the subdivision with the proposed well and septic sites. There is an existing well located on proposed lot 6. We are proposing that two additional wells be drilled and a well drawdown test be completed using these three wells. We are proposing the well located on Lot 6 be inspected and documented. If the well is found to not meet MOA well ordinance requirements, we would propose a new well be drilled next to this one and the existing well be abandoned per MOA code. We are proposing a well be drilled on Lot 5 approximately 200 feet northeast of the well on Lot 6. We are also proposing that a th/rd well be drilled on Lot 4 which is approximately 140 feet nm~da/northeast of the well on Lot 5. We will require all wells to be surveyed and staked prior to drilling to insure proper separation distances to the proposed platted reserve sites. These well locations were chosen for the accessibility for a well rig. All other lots would require extensive grading and driveways to be installed in order to drill wells on any other lots. We request three exploratory well permits be issued for the purpose of perform'mg an aquifer test (see attached permit applications).
