SUBJECT: Tract C, Cinerama Terrace Subdivision SW010246 The permit to construct the well and septic system on Tract C, Cinerama Terrace Subdivision was issued on July 18, 2001. The well was placed on July 27, 2001. The septic system however was not constructed until November 15, 2002 after the permit had expired. We have recently paid the fees to renew the permit from July 18, 2002 until July 18, 2003. The owner of the property decided in mid 2002 to place an Advantex Treatment System in lieu of the previously designed and permitted Bottomless Sand Filter. The design change was discussed with Dan Roth of your staff, but no documentation was submitted to your office. I have now attached the modified septic system plan, which was originally completed in June of 2002. The new system was installed in accordance with the attached plan and under the supervision of Anchorage Tank, Orenco Systems and myself. We are therefore applying for a permit to construct a new well and septic system on the lot to serve the new home. The attached Site Plan and backup documentation identify the location and configuration of the new septic system and the. parameters used in the design. Also identified on the plans are the location of the well along with the test holes and the alternate absorption site. Existing drainage patterns are shown and will not be altered by the development of the lot. 'We have surveyed the area and the 'new system will not conflict with other wells or septic systems in the area. Substantial effort has been made on this tract to control both the surface and subsurface drainage. Several french drains and ditches have been placed on the property at the locations shown on the Site Plan. Groundwater observations over the past two years indicate the drains are effective in keeping the water a minimum of 4' below the bottom of the existing surface. The area for the septic system was previously cleared and monitored carefully to determine groundwater intrusion. We are now confident groundwater encroachment will not affect the new bottomless sand filter and should provide a minimum of 6' of separation from the highest groundwater level. Two test holes at the location of the new absorption bed indicate poorly graded sand with varying degrees of silt. Percolation tests in the material indicated rates approximating percolating at rates less than 1 minute per inch. The material underlying the sand is a silty gravel which percolates closer to 50 minutes per inch. We are therefore proposing to place a bottomless sand filter system on the lot at the location shown. The absorption bed will be 12' wide and 30' long for a total absorption area of 360 square feet. The ground surface on the lot slopes as shown on the attached Site Plan with grades from east to west that flatten in the area of the proposed absorption bed. The new absorption bed will be constructed on a fairly flat surface as much as possible in conformance with Municipal requirements. The existing drainage pattern on the lot will not be affected during development. DATE: 72-(X~ (Rev. 4/85) ON-SITE WASTEWATER DISPOSAL SYSTEM CONSTRUCTION AND MATERIAL SPECIFICATIONS SUBJECT: TRACT C, CINERAMA TERRACE SUBDIVISION r GENERAL: -1. The scope of this project includes the procurement and placement of a new 2,000 gallon combination septic tank and lift station and pressure distribution system. Work also includes the construction of a new 30' long x 12' wide Bottomless Intermittent Sand Filter at the location shown on the attached Site Plan. A minimum of 2' of coarse sand meeting the specification from Anchorage Tank must be imported and placed in the filter area. The 6" of pea gravel and distribution piping must then be constructed atop the imported filter sand material in conformance with specifications fumished by the supplier. The distribution piping for the filter must be placed 2.5' above the original ground surface at the area shown on the Site Plan to provide a minimum of 4' separation to groundwater. All work must be completed in stdct conformance with the manufacturer's requirements for installation of the intermittent sand filter. Construction shall be in accordance with the approved site plan, design drawings, Municipal Permit with any special provisions or conditions, and all applicable State and Municipal Wastewater Disposal Regulations. The Contractor shall be responsible for obtaining all underground utility locates and for the layout of the septic system and verification of the location of all lot lines. Unless specifically agreed otherwise, the contractor shall be responsible for final grading areas subsequently depressed from soil settling. Property owner shall be responsible for revegetation of affected areas unless specifically agreed otherwise. Contractors installing wastewater disposal systems must be certified by the Municipal Department of Health and Human Services for system installations. Owners installing their own systems must receive pdor approval from D.H.H.S. before beginning system installation. SEPTIC TANK/LIFT STATION INSTALLATION 1. The 2,000 gallon septic tank/lift station must be constructed by a certified tank manufacturer. Construction shall include an 18" manhole to provide access to the lift station. 2. The septic tank shall be sufficiently bedded to prevent settling or shifting of the tank.' 3. All standpipes on the septic tank shall extend a minimum of 12 inches above final grade. 4. Tanks installed without 4' of cover shall have a minimum of 2" of direct burial insulation. '~ract C, Cinerama Terrace June 27, 2001 Page 2 of 3 A foundation cleanout shall be installed one to four feet from the building foundation. Two cleanouts are required between the tank and the drainfield. Final grading over the tank/lift station shall be such that a positive slope exists away from the septic tank. ABSORPTION BED CONSTRUCTION: 1. The absorption bed shall be constructed to the dimensions shown on the design, The bottom of the bed shall be within 2" of level. Distribution piping must be placed level with perforations down atop a level bed of drainfield rock. Rock should then be placed over the pipe to provide a minimum of 2" of cover. 3. A silt barrier or geotextile fabric must be placed between the drainfield rock and the natural soil backfill. 4. Monitor tubes must be 4" in diameter and installed at the locations shown on the design. The portion below ground must be perforated. Contractor shall vedfy the septic tank and drainfleld are a minimum 100' away from any private water wells in the area, 150' from a Class "C" Well or 200' from any community well. o Direct bury insulation must be placed over the distribution system if less than 3' of backfill depth is available. Finish grade over the trench must be mounded to prevent settlement or depressions. 7. Grade area surrounding the absorption trenches to drain away. A minimum 2' of accepting soil is required below the drainfield rock for a 5' wide trench. Contractor shall redly this condition prior to placement of the rock. All pockets of unacceptable materials must be removed and replaced. MATERIAL SPECIFICATIONS: The lift station must be constructed by a Municipally approved septic tank manufacturer. An Orenco 20 OSI 05 HHF-5 pump is recommended. Anchorage Tank is a recommended supplier for the materials required for this project. 2. The following pipe materials are approved for use in septic system installations in the Municipality of Anchorage: Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated and solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and ASTM D2662 or A.B.S. (perforated and solid). 2. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical Co. Styrofoam HI or equal). Tract C, Cinerama Terrace June 27, 2001 · Page 3 of 3 3. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Fernco, or equal). 4..A permeable geotextile fabric (Typar, Mirafi or equal) must be installed between the final drain rock layer and the native soil layer. 5. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing the #200 sieve. EQUIPMENT SPECIFICATIONS: Air Supply Line shall be a Wasteflow Er~itterline, %" .inside diameter from Anchorage Tank. Line shall be insulated with.%" foam pipe wrap inside a 2" Schedule 40 PVC Jacket buried a minimum 3' below the surface. Air Compressor shall be a Thomas Industries Model 5070 as supplied by Anchorage Tank. Compressor shall be located in the garage or crawIspace. A pressure gauge calibrated from 0 to 10 PSI shall be fumished and installed in line with the air line. The gauge shall be mounted in an area which provides easy viewing and is seen on a regular basis such as the entrance to the home. Material for Filter Sand must meet the following gradation: 50% or More Passing the No. 4 Sieve 2% or Less Passing the No. 100 Sieve 1% or Less Passing the No. 200 Sieve INSPECTIONS: A minimum of two inspections are required by Municipal Ordinance. These inspections must be conducted under the supervision of a professional engineer registered in the State of Alaska. The first inspection must be conducted after the excavation of trenches, beds or pits and before the installation of any gravel. A septic tank may be set in place, but may not be backfilled. The second inspection must be conducted after the placement of the geotextile fabric, gravel, distribution piping, standpipes, cleanouts and insulation. No backfill should be in place at the time of inspection. Contractor shall provide a copy of all field survey layout and construction notes for use in preparing the certified as-built of the completed system. In consideration of the mutual covenants contained herein, the parties to this Memorandum of Understanding agree as follows: 1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality grants permission to Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS), described as Anchorage, Alaska. 2. Definitions. Any change to the design or function of an AWWTS that includes the installation or removal of any parts, components or pieces not included.in the original eonstruetion permit and design. Prior to performing any alterations to an AWWTS the owner must obtain a Wastewater Disposal System Construction Permit from the Municipality pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65 Certificate of On-Site Systems Approval. An approval by 'the Municipality of existing water and wastewater disposal systems given at the time of property sale and title transfer in accordance with AMC Page 1 of 5 4. Term. The term of this Memorandum of Understanding shall be for the life of the AWWTS. The term begins on the date of approval by the Municipality of the installed system and shall continue while the AWWTS system is in use or operational or until the property is sold or title is transferred by owner and a new certificate of on site approval is issued to the new owner or transferee of the property. 5. Alterations, Installation and .Removal of Additional Equipment. Owner agrees not" to make any alterations, removal of parts or additions to the AWWTS without a Construction Permit froTM the Municipality. .. 6. Maintenance and Repairs. A. Throughout the term of this Memorandum of Understanding, the~ Owner. shall maintain .AWWTS in good repair. In addition, it shall be the responsibility of the Owner during the term of this Memorandum of Understanding, and any extensions or renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs..Further, owner agrees to comply with all applicable ordinances, laws,.regulations, rules and orders for the AWWTS. B. Owner agrees to provide the Municipality a .written schedule of routine maintenance and repairs which have been performed on the system pursuant to the terms and conditions contained in the Owner's AWWTS Operating Permit. This schedule shall be submitted to the Municipality annually upon :the renewal of the permit.
