Subject: SEPTIC TANK PERMIT APPLICATION
Cohoe Lot 3
I am submitting an application to upgrade the septic tank for this lot. The existing septic tank serving the
above referenced lot has been compromised and the owner would like to replace it. The owners wish to
replace the existing 1000 -gallon steel tank installed in 1996 with a 1000 -gallon HDPE tank. The
absorption field passed an adequacy test and is functioning well. The submittal consists of one (1)
drawing showing the present improvements on the lot and the proposed improvements of the lot, of
which only the septic tank is subject to this permit application. Attached is also an existing and current
as built survey from 2016 in the On -Site Department's file. There have been no changes to the lot since
that survey.
The installation of this septic tank will not prevent wells and septic systems from being installed on the
adjacent lots. There are no developed or natural surface / sub surface drainage courses within 100 feet
of the proposed septic tank location. The proposed septic tank will not change the general slope of the
area. Ponding and/or concentration of surface runoff will not result from this installation.

In conducting this evaluation, GEG provided an engineering evaluation of the well and/or septic
system in accordance with the guidelines and regulations established by the Municipality of
Anchorage and industry practices. The reported results describe the condition of the system/s on the
date/s of the evaluation. Separation distances were measured to readily identifiable features.
Hidden defects or encroachments may exist that were not identified during the evaluation. The
operational life of all wells and septic systems depend on a variety of variables including, but not
limited to, soil conditions, groundwater levels (that may fluctuate during the year), quality of
construction (materials and workmanship), and the water usage of the family utilizing the system/s.
These conditions can vary, and are outside the control of GEG. Satisfactory test results do not
guarantee future performance of the system/s; therefore, GEG makes no warranty (express or
implied) regarding the future performance of the well or septic system. GEG makes no
representation whether an alternative well or septic system can be installed on the property in the
event either of the current systems fail. The content of this report is for the sole benefit of the
person/party who retained GEG. Reliance upon the information provided in this report by any other

To whom it may concern:
The existing septic system was installed on the subject property on 9/17/85. The sump is approximately
9.5 feet away from the foundation of the existing home. Per the 1985 inspection report, the drainfield is
approximately 2.5 feet wide resulting in a separation distance of approximately 8.25 feet. Below are a list
of justifications as to why a waiver should be granted for this encroachment.
Encroachment has existed for 32 years.
The basement is approximately 5 feet below grade, however, the top of drainrock (in the
drainfield) is approximatley 9 feet below grade. The drainfield would have to be surcharged by
approximately 3' and backing up into the basement plumbing before it could laterally migrate
over and into the basement.
At this time, we request that your department grant a waiver for a separation distance of 8' from the
foundation to drainfield.
