Dear Steve:
Recently I did an inspection on this lot in which a new septic tank was
installed to replace an old damaged tank. There was no permit issued to do
this, but the excavator explained that you had told them a permit was not
needed, which you confirmed in phone converation with me on 10/27/86.
In replacing the tank, the excavator placed the new tank exactly where the old
one was, which unfortunately is only 96 feet from the well. They did not
notice the distance because the well is on the other side of the house.
Ground elevation of the well is slightly higher than at the tank. Calder
couplings were used on the tank and it does not appear there will be any
danger of contamination from the tank.
Therefore we are requesting a waiver of the well to septic tank separation
distance to be only 96 feet for this lot.

Septic System Upgrade
To Whom It May Concern:
Attached please find the application and supporting documentation for upgrading the
septic system for the existing 4 bedroom house on the referenced lot. The existing
septic system was installed in 1971, with an upgrade in 1986 (new tank), and it has
failed. A trench was added to the system, but was not permitted or documented. It is
proposed to document the existing trench and retain it for future use. The house was
previously used as a bed and breakfast, under Alaska Department of Environmental
Conservation jurisdiction, but will now be a single family residence.
The proposed design is based upon a test hole dug on May 19, 2004. It was dug to a
total depth of 18 ft with no water encountered while digging or after 7 days of
monitoring. A percolation test was run between 5.5 ft and 6.0 ft (not at the bottom of the
proposed trench, due to safety concerns), with the result of 9.6 minutes/inch. The soil
log is attached.

The current septic tank will be abandoned in place. The crib will be retained, and we
Propose to document the existing drainfield and retain it for future use. Documentation
of the trench will include the following:
1. Inspect the condition of the trench by digging down to the top of the sewer rock
(at about 3 ft from the surface). Determine the depth and ensure that there is a
fabric barrier between the rock and the backfill material. Determine the length of
the trench by exposing the west end.
2. Dig a test hole within 30 ft of the trench to a depth 6 ft below the bottom (17 ft).
Log the soils and perform a percolation test, to confirm that the trench is properly
sized for actual conditions. Monitor groundwater for 7 days.
3. Prepare an as -built drawing of the trench.
If the trench does not meet code requirements, it will be abandoned in place.
There are no surface waters or private wells within 100 ft of the proposed septic system.
I am not aware of any adverse effect that this upgrade would have on any adjacent
properties' water and wastewater treatment needs.

in the CerUficate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply andlor wastewater
disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated herein. I further verify that
based on the Information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply
and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes, ordinances, and regulations in effect at
the time of installation.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA COSA
guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test,
and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend,on the local soil
condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions
are outside the control of the evaluator of this system. All systems eventually fall and satisfactory test results do not guarantee future
performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provide any warranty
for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed
above.

A+ Home Services repaired the septic system on the above referenced property on October 3, 2016. They exposed
the underground flow splitter and found that the line to the upper trench had reverse grade. The line between the
tank outlet and flow splitter was lifted to make positive drainage possible to the upper field from the flow splitter to
the field. Currently both trenches have positive drainage towards them. The flow to the lower saturated field has
been cut off so it has a chance to recover. Even distribution will be started after a period of three months.
It is my opinion that this system is operating adequately for a four-bedroom system.

To whom it may Concern;
I am writing in response to your comments dated September 26, 2016 concerning the above referenced lot,
specifically addressing comments 3 and 4. These comments are concerning the condition of one of the two fields that
comprise the soil absorption system.
There are two fields that comprise the soil absorption system for the 2004 system, which we tested. The lower field
was saturated when we arrived on property. The upper field was dry. Water was added to the double clean-out to
determine how the field operated under normal use. The water levels in the lower field rose 30 inches with the
induction of approximately 100 gallons, while the upper field stayed dry. The test was paused at that point and
continued with the water being introduced directly into the upper field. 533 gallons were introduced into the upper
field, which rose the water levelsfive inches. The water levels returned to the starting levels within 365 minutes. ThE
conclusion is that the lower field is in failure,while the upper field is working adequately for a four bedroom house.
The effluent is not splitting evenly between the two fields. The entire system is approximately 50% used. Of note is
that there is an approved four bedroom system lying dormant adjacent to this soil absorption system and connected
via a diverter valve.
It is my opinion that this system is operating adequate for a four bedroom system.

Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority Approval are
valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued with
new water sample results. (Certificates may be reissued for a period of up to one year with valid water samples.)
Certificates are valid for one year for properties served by Class A or B wells or a public water system. The
Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.
