RE: Proposed Septic System for JEM Lot 3
Dear Reviewer,
The above referenced property is currently served by an older septic system with a leaking tank that
needs to be replaced.
The owner wishes to replace the tank at once.
Our review of available documentation and field investigation show that this project will not adversely
impact any nearby Wells, Wastewater disposal systems, replacement disposal sites, or drainage flowing
onto and off of the subject property.
As shown on the plan, the tank will be greater than 10’ from the house foundation.
The tank shall be covered with a minimum of 2” moa approved insulation and 3’ of cover or a minimum of
4’ of cover without insulation.
The repair must be performed by a moa certified installer in accordance with MOA requirements.
Repair of the proposed system will not negatively impact adjacent lots.
Upon completion of the installation, a record drawing will be submitted showing the location of the new
tank, leach field, well, and other applicable features.

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The existing absorption trench on the subject lot has failed and must be replaced .prior
to the issuance of a Certificate of On Site System Approval (COSA). We are therefore
requesting a permit be issued for the upgrade of the septic system to accommodate the
three-bedroom home on the lot. The_ attached Site Plan and backup documentation
identify the location and configuration of the existing and proposed septic system. The
existing Septic Tank will be uncovered, inspected and verified suitable for continued
use. The existing absorption trench will be left intact and tied to the new system with a
flow diverter valve. Also identified on the plans are the locations of the existing well on
this and adjacent lots and adjacent septic systems. No conflicts exist between the
proposed septic system and the wells on this or adjacent lots. Protective radii are
shown to the adjacent wells. Drainage arrows are shown indicating the current
drainage patterns. The drainage patterns will be maintained after construction.
The test hole placed on the lot during the initial installation indicated Silty Sand to well
graded sand (SM/SW) to a depth of 18'. No groundwater was encountered and none
developed during the monitoring period. The percolation rate in the SM soil lense was
determined to be 4.4 and 2.2 minutes per inch in the SW lense. We have designed
the new trench with an application rate of 1.2 gallons per day per square foot. We are
proposing to place a 40' long by 5' wide absorption trench with a 4' effective depth. The
total depth of the system will be 7' with the distribution line at 3' below the existing
surface. A flow diverter valve will be placed to connect the existing trench to the system
for future use. The existing system appears to be in the electrical easement. We are
requesting the permit be issued for construction of the new trench while the
encroachment permit for the existing trench is ongoing. If we are unable to acquire the

encroachment permit in a timely manner the existing trench will be abandoned in place
in accordance with Municipal Code.
The ground surface on the lot slopes to the northwest in the area of the absorption
trench. The trench will be placed parallel to the contours of the ground as much as
possible and in conformance with design criteria. A minimum of 100' will be maintained
between all components of the septic system and the proposed well and from any
surface water in the area.
If the system is constructed in accordance with our design the following statements
apply:
1. The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in theprofessional engineer's work.
