Roth, CiwL1 Engineer, On-site Servi~ Department of Health and Human Services Request for Comments on Subdivision June 19, 1991 The Environmental Services Division, On-site Services Section, of the Department of Health and Human Services has reviewed the following cases and has these comments: S-8405C: Lots 1 - 16 Shelly Marie Estates Subdivision Extend public sewer and water to all lots. S-85-041: Edwards Manor Subdivision Sanitary Sewer Agreement Extension No objection. S-9074: Edgewater Subdivision Extend public sewer and water to all lots. S-9075: Eagle Heights Subdivision Provide report prepared by an engineer or (groundwater) hydrologist which considers impact on existing adjacent wells and long term capacity of the producing aquifer to meet the water demands of the additional 12 lots. Well logs from three (3) test wells in the proposed subdivision and well logs from existing adjacent wells suggest that production capacity of the bedrock aquifer may be limited. Extend public sewer to each lot~ We will require a plat note indicating that on-site sewage disposal may not be used on any lot. DJR/ljm:381 Platting Board July 3, 1991 Page 3 Summary of Action Providing a report from a registered engineer, reviewing the existing soils conditions, subsurface water levels, and surface drainage with recommendations on street construction and building foundations in relation to the findings in the report. Resolving street names and designations with Public Works and the Post office prior to initiating a subdivision agreement. Placing a note on the plat that the bottom of the foundation footings for all lots shall be no lower than eighteen inches'below the curb of the street. Providing an engineers report ooncernlng the presence of absence of discontinuance permafrost with recommendations for development if any is found. 10. Placing the following notes on the plat: a. "Lot one, Block one, shall have no direct vehicular access to 64th Ave." b. "All corner lots shall have driveways located a minimum of 40' from edge of intersecting roadways." c. "Direct vehicular access from any corner lot to more than one street is prohibited." d. "Direct vehicular access from ]Lots 9 and 21, Block 3 to Newt Drive is prohibited." e. "Direct vehicular access from Lot 1, Block 3, to Norm Drive is prohibited. 11. Placing a note on the plat concerning the floodplain in accordance with 21.15.020 (F). 12o Develop an acceptable phasing plan with the Department of Public Works to develop subdivision into two phases of approximately equal number of lots, one year apart. Request for Comments on Subdivision Cases February 20, 1991 The Environmental Services Division, On-site Services Section, of the Department of Health and Human Services has reviewed the following cases and has these comments: 88-5(S-8154: Essex Square Subdivision Time Extension No objection to time extension. 89-2: Dowling Subdivision Time Extension No objection. S-9020: S-9021: Lots 1 7 Saugstad Subdivision These lots'will require public sewer. Applicant must provide DHHS with informatio~ on water availability for private well development. Lot 1 6 Parkridge View ~{ates Subdivision DHHS has received no additional soil tests since the last submittal in 1985. These soils show the area to be underlain with shallow groundwater and poor soils. The tests fail to comply with current code requirements for water monitoring and percolation testing; not one of the tests is acceptable for on-site disposal. There is a large glacial build up in the original road cut along the south border of proposed Lot 1, indicating again a shallow groundwater table existing on this property. The developer needs to provide the department with proper soil tests, water availability information for private wells, and a master drainage plan. Applicant must also decide which reserved area provision he wishes to utilize in development. DHHS will require a 100' protective setback to the stream be shown on the final plat. Based on observations and soil information at the site and nearby, it is questionable whether this land is suitable for development. We strongly recommend that the plat be denied as submitted, or returned to the engineer for additional information.
