The subject permit, issued October 16, 1992 by this office for a
single family well and/or on-site wastewater system, has
expired as of October 16, 1993.
A new permit must be obtained from this office for a well
and/or on-site wastewater system NOT installed by' the
expiration date.
If you have drilled the well, a well log must be sent to
this office for documentation of the installation and to
close the permit.
If a licensed Professional Engineer has inspected the
installation of the on-site wastewater system, the original
as-built inspection report must be sent to this office for
review, approval and documentation. All inspection reports
must be submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $320.00 for an
on-site wastewater permit; $120.00 for a well permit and
$440.00 for a combined on-site wastewater and well permit.

Lot 1, Chugach Gardens Subdivision
Septic System
The proposed system is tp provide a sewage system for a 4 bedroom
home on the subject lot. There is currently an old septic system (size
and type unknown) serving a trailer on the lot. The trailer is being
removed and the septic tank will be pumped, filled with soil, or crushed
and buried.
The soils encountered were a poorly graded gravel/poorly graded
sond with some silt. The percolation rate was 1.1 minutes per inch,
and no ground water was found to a depth of 16 ft.
The lot is nearly flat but drains well due to the type Of soil.
The system will have no measurable impact on wells and septic
systems on neighboring lots; reserved space, surface or subsurface,
or on drainage.

i am writting this letter to inform you that the house
we bought, at ~16~ 13 1-'arksville ~d. is not a ~;'uple×, It was
at one time, one half of a duplez., zero lotline, style ~'own-
house. The two homes were split in half and sol5 to private
contractors. '±'his house came off the site on the Eagle River
Loop t-~d. where several homes were removed to accomidate the
~ighland ~.d., & the new Eagle ~iver ~.~ri~ge. We are now purchas,~
lng our half from Jay Williams, who also owns the other half.

SUBJECT: Lot 1 Tract A Chugiak Gardens Subdivision
A permit issued by this Department for an individual well
and/or on-site sewer system has expired as of December 31,
1984.
Permits are issued on a calendar year basis by authority
of Municipal Ordinance. A new permit must be obtained from
this Department for any well and/or on-site sewer system not
installed by the expiration date.
If you have drilled the well, a well log needs to be sent
to this Department for documentation of the installation
and to close the permit.
If a private engineer inspected the installation of the
on-site sewer system, the original as-built inspection report
and the yellow copy must be sent to this office for review
and approval, and for documentation.
