Dear Ms. Romer,
Attached is a copy of the on-site s~ptic inspection re~0rt which
has been approved by the Municipality of Anchorage (MOA), Department
of Health and Human Services (DHHS) for the referenced property.
We have also performed an inspection on the well serving the referenced
property. The well is located more than 100 feet from any contamination
source, the ground slopes away from the well and the well is equipped
with a sanitary seal. We connot take samples to be tested for water
quality (bacteria and nitrates) until a pump is placed in the well
and the well is placed into operation.
A Health Authority Approval (HAA) cannot be obtained until a house
is built and the water and septic systems are placed into operation.

Lot 1 Bear Park Subdivision
A permit issued by this Department for an individual well and/or on-site
sewer system has expired as of December 31, 1985.
Permits are issued on a calendar year basis by authority of Municipal
Ordinance. A new permit must be obtained from this Department for any
well and/or on-site sewer system not installed by the expiration date.
If you have drilled the well, a well log needs to be sent to this
Department for documentation of the installation and to close the permit°
If a private engineer inspected the installation of the on-site sewer system
the original as-built inspection report(three part form) must be sent to
this office for review and approval,and for documentation.
