This approval applies to the existing on-site wastewater disposal

Request you issue a permit to install septic system to serve the proposed three
bedroom dwelling on the referenced property, and grant a waiver for the
separation distance between the existing leachfield at 0 feet (crosses over), and the
proposed trench at 1 foot. This will not prevent a septic upgrade on the adjacent
property. Also, request a waiver of the required 20 foot separation between the
existing trench and the proposed trench down to 15 feet separation. This will
avoid the need for a waiver to surface water.
A test hole was excavated and a percolation test was performed. The approximate
location of the test hole is located on the attached site plan.
At the time of excavation, September 30, 1998, groundwater was not encountered in the
test hole. After seven day groundwater monitoring, the monitoring tube was checked
and was found to be dry..
We do not anticipate any adverse effects on neighboring wells, septic systems, reserve
areas or drainage patterns by the installation of the proposed septic system. The
construction of this system will not prevent any future development on any of the
adjacent properties.

GENERAL:
The scope of this project includes the installation of a 1300 gallon polyethlene septic
tank, three bedroom leachfield trench to serve the residence located on the referenced
property.
Construction shall be in accordance with the approved site plan and design drawings,
Municipal permit with any special provisions or conditions, and all applicable State and
Municipal Wastewater Disposal Regulations.
The contractor shall be responsible for obtaining any necessary underground utility
locates.
Unless specifically agreed otherwise, the property owner shall be responsible for final
grading areas subsequently depressed from soil settling.
Contractors installing wastewater disposal systems must be certified by the Municipal
Health Department for system installations. Owners installing their own systems must
also receive prior approval from the Municipal Health Department.
SEPTIC TANK INSTALLATION:
A septic tank is to be constructed by a certified septic tank manufacturer. Construction
shall include two 4" cleanouts for pumping access.
The septic tank shall be sufficiently bedded to prevent settling or shifting of the tank.
All standpipes on the septic tank shall extend a minimum of 12 inches above final
grade.

Septic tanks installed with less than 4 ft. of cover shall be insulated.
A foundation cleanout shall be installed one to four feet from the building foundation.
In the line between the tank and the leachfield there shall be two adjacent cleanouts
(unless an effluent pumping system exists within the septic tank). These cleanouts shall
be located on undisturbed soil not more than 10 ft. from the tank. The first cleanout, in
line, shall be to clean toward the leachfield. The second cleanout shall be to clean
toward the septic tank.
Final grading over the septic tank shall be such that a positive slope exists away fi:om
the septic tank.
ABSORPTION TRENCH/DRAINFIELD INSTALLATION:
Excavate the proposed trench to the dimensions shown on the design. The bottom of
the excavation shall be within 2 inches of level. If the sidewalls of the excavation
become smeared, they must be raked or scratched (ruffed-up) before gravel (sewer rock)
placement.
Once the gravel is installed, the distribution pipe is to be installed level with the
perforations faced downward. Gravel is then to be placed over the distribution pipe to
provide a minimum of 2 inches of cover over the pipe.
A silt barrier must be installed between the final gravel layer and the native soil backfill.
Ensure the silt barrier covers the entire gravel surface before placing backfill.
Monitor tubes shall be of four (4) inch diameter, installed approximately in the
locations shown on the design, and extend a minimum of 12 inches above final grade.
The portion of the monitoring tube extending through the gravel shall be perforated
from the bottom of the trench to the invert of the distribution pipe. This is equivalent to
the effective depth of the gravel as noted on the design.
Backfill over the final gravel layer must not be less than twenty-four (24) inches.
Insulation must be installed when the backfill depth is less than thirty-six (36) inches.
The finish grade over the trench must be mounded to prevent the formation of a
depression after settling.

MINIMUM MATERIAL SPECIFICATIONS:
Any septic tank proposed for installation must be constructed by a Municipal approved
septic tank manufacturer.
The following pipe materials are approved for use in septic system installations in the
Municipality of Anchorage:

Use of a type of pipe other than listed above must be approved by the inspecting
engineer.
Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical
Company Styrofoam HI or equal).
Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Femco,
or equal).
A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N, or equal) must be installed
between the f'mal leachfield gravel layer and the native soil backfill.
All leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel with less than 3%
passing the #200 sieve.
When sand is being used as a filter material, its gradation specifications must conform
to current M.O.A. or D.E.C. requirements, which ever requirement applies.
INSPECTIONS:
Typically there will be a minimum of three (3) inspections required during the installation of
the wastewater disposal system. These inspections will occur as follows:

The first inspection must be conducted after the excavation of ditches, pits,
trenches, or beds and before the installation of any gravel. A septic tank may be
set in place, but may not be backfilled before this inspection.
The second inspection must be conducted after the placement of the silt barrier,
gravel, distribution lines, standpipes, cleanouts, and insulation, but before the
placement of any other backfill.
3. The final inspection is to occur upon final grading of the property.
Often there will be more than these 3 inspections required. Especially with the installation of
multiple trenches, sand filters, pressurized distribution systems, etc. Thus, the inspecting
engineer is to be contacted at least 24 hours prior to the start of construction. If necessary, a
pre-construction meeting will take place on-site. The inspecting engineer will not coordinate,
direct or control in any way the contractors activities.
The owner shall contract with the contractor to perform the work outlined in these
specifications and plans and in accordance with the attached M.O.A. permit. There will be no
contractual arrangement existing between the contractor and S & S Engineering. S & S
Engineering shall be the owner's representative and will inspect the work as stated above to
document the contractors activities. Final acceptance of the contractors work rests with the
owner and the M.O.A.
S & S Engineering shall have no liability to the owner or to others for acts or omissions of the
contractor or ax~y other persons performing work on this project or the failure of the contractor
to carry out the work in accordance with these construction documents. S & S Engineering's
inspecting engineer will not be responsible for the construction means, methods, techniques,
sequence, procedures or the safety precautions incident to this project.

Dear Mr. Barter:
The septic system on the subject lot was installed at a time
when existing code required a 50 foot separation between a
private well and any component of a septic system (tank or
absorption field). The septic system on the subject lot was
therefore in compliance with the code requirements in effect at
the time of its installation in August of 1967. A waiver of
the current 100 foot separation requirement is therefore not
required.
This Department concurs with your assessment that the existing
separation distances on the subject lot adequately protect the
well from contamination given the depth of the well and the

ydrogeologlcal conditions in the area.

Dear' Sir:
At the request of Hr. Leroy Jeffery, I visited the subject
property on Plovember 22, 1986 for the purpose of performing an
adequacy test on tire existing septic system. The results of the
test were satisfactory as was the coliform lab analysis. Upon
reviewing the as-built drawing dated April 25, 1984 and
plotting the missing features, I found the following: The septic
system does not meet. current separation distance requirements
to the well. As shown by the attached notarized statement, the
system was legal upon installation. Also , the trench crosses
onto Lot 14 as shown on the attached as-built. I researched the
Itunicip~iities files end found no records in conjunction with the
subiect property.
Therefore the following has been accomplished iii pursuit of a
separation waiver and tiealth Authority Approval. A notarized
statement from the previous land owner has been provided which
would substantiate grandfather rights to the separation
distal'ices. This distance is measured at 89 feet fi-om the well
cap to the western most standpipe of the tank. I am requesting e
85 foot separation waiver. A right-of-entry agreement is
attached indicating the physical acceptance of the system
between 8djacien[ land owners.

Finel]y, I have provided a review of the geology and
geomorphology of the surrounding oreo. The rolling terrain in
the immediate area slopes towards Sundi Lake. ~ell depths
solicited ~he existiog drillec~ who performed worn in ~he area
during the late 60'~ and ~he ~epor~ment of ~t~rol
~one of these contacts could produce o well log for the
prope~y. I did verify ~hro~gh "3D Pump and ~ell~ ~h~ the ~ell
would be approximately 200 fee~ deep end c~ed throughout
length. They hod been on-site ~o perform maintenance o~ the
· ~irhlg. The ~el] cap faf Lo~ 1~ is approximately 15 fee~ higher
vertically than ~he septic ~ystem. The soil~ are composed
vo~'ying s~ra~a~ of ~and~, gravels, ond di~inct clay layec~. Of
particular notice i~ the impervious layer of clay and hard pan
for Lo~ 14 ~hich i~ found at the 164 to 1~3 fee~ belo~ original
ground.
Tile septic systems installed on adjacent lots indicate a surface
layer of 125 sqaare feet per bedroom soils for installation of
absorption systems. There is no apparant water table near the
surface.
Based on subjective deduction, I would say that this system is
composed of a two compartmeot tank with a capacity of 1:300 to
1500 gallons (pump receipt). The leach field appears to he
composed of 5 feet of gravel approximately 57 feet long ar 570
sqo~re feet of capacity. On the date tested the system would
accept 680 gallons per day.
It iS rely opinion, WlliCh is based on a review of the previously
stated information , that this system currently is ~nd will
continue to operate in a safe and adequate manoer.

AGREEMENT OF ENTRY
This agreement, made and entered into this /~ day of
· December, 1986, by and between Bonnie L. McGee and Wilbert D.
McGee, husband and wife, hereinafter referred to as "licensor" and
Leroy Jeffery, hereinafter referred to as "licensee", witnesseth:
'That for add in consideration of the sum of ten dollars
($10.00) and other good and valuable consideration herein
acknowledged as received, Licensor hereby grants to licensee the
right, privilege and permission to maintain that part of a
sewage drainage field which commences upon Lot 13, Block 1, Sundi
Lake Subdivision, Grid 2224, Municipality of Anchorage, Alaska,
(property owned by licensee) and appears to extend and be partly
located upon Lot 14, Block 1, Sundi Lake Subdivision, Grid 2224,
Municipality of Anchorage, Alaska, property owed by licensors.
The said license is personal to the licensee, shall not inure to
his successors or assigns and shall terminate upon a resurvey of
the common boundary line separating said Lot 13 from said Lot 14.
