Lockman 8235 Endicott Street Anchorage, Alaska 99502 Subject: Lot 5 Block 2 Sundi Lake Subdivision Approval for your individual sewer and water facilities can not be granted until the following items have been (1) Exp6se the well for our inspection to determine proper construction, also to insure the minimum distance requirements are met b~tw~en your well and~ewer system. The water analysis report be delivered to this office from Chem Lab, 5633 B Street, for our review. The septic tank pumped with a receipt submitted to this office. A four(4) inch cast iron cleanout needs to be installed in the septic tank and/or leaching area. A percolation test be performed on the existing leaghing area. This test will determine if the system is adequate according to National Standards. A listing of private firms performing the test is enclosed.
