At the time of construction unsuitable soil conditions were encountered. A new test hole was excavated and better soils were found. The drainfield will be designed around the 30 foot radius of this test hole. We are proposing that a new 1000 gallon septic tank and deep trench type drainfield be installed. Comments regarding the design, are summarized as follows: 1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring, and the percolation test results. It is our opinion that due to the overall appearance of the soils, an application rate of 1.0 gallons/day/fie should be used. 2. TRENCH DESIGN: a. Percolation Rate: 2.5 minutes/inch b. Proposed Application Rate: 1.0 gallons/day/ti2 c. NumberofBedrooms: 3 d. Design Flow: 450 gallons per day e. Minimum Absorption Area: 450 fl2 f. Total Depth: 12 feet (max.) g. TOPOGRAPHY: As can be seen on the attached topography site plan, the average topography in the area of the proposed drainfield is a 10-15 percent slope running approximately southwest to northeast. A new permit must be obtained from this office for a well and/or on-site wastewater system NOT installed by the expiration date. If you have drilled the well, a well log must be sent to this office for documentation of the installation and to close the permit. If the on-site wastewater system has been completed and a licensed Professional Engineer has inspected the installation of the on-site wastewater system, the original as-built inspection report must be sent to this office for review, approval and documentation. I spoke with you on Thnrsday, 8/5/93, by telephone regarding the maximum on-site sewer system depth. The test hole locations were excavated to a depth of 16 feet which will determines the maximum depth of the system to be i0 feet below grade. The enclosed design has located the bottom of the system at 12 feet below grade. In our conversation you stated that the Department of Health and Human Services would conditionally issue the well and septic permits with the stipulation that the test holes be excavated to a depth 6 feet below the bottom of the systen~ In the case of this on-site sewer system it would mean excavating to a depth of 18 feet below finish grade. Probable Impact Narrative Legal Description: Lot 12, Block 2, Stmdi Lake Subdivision Well Impacts The 100 foot radius will arc into Lot 11 approximately 12 feet at the deepest penetration point. This should have minimal impact as Lot 11 has been previously developed with a septic and well sites. Subsurface Disposal System Imoacts Lots 3, 4, and 5 of Sand Lake Subdivision, to the north of Lot 12, will not be able to place a well within 20 feet of the southern property line. This does not appear to be a significant impact as Lots 4 and 5 have wells located on the northern portion.
