Bragaw Street P. O. Box 196650 Anchorage, Alaska 99519 Subject: Sun Valley Heights S/D, Block 1, Lot 6 Conditional Certificate of Onsite Approval Request Septic System Permit Request Ladies and Gentlemen: I am writing to request a permit to construct a new septic system be issued for this property and a conditional Certificate of Onsite Approval (COSA) be issued. The proposed systems will serve an existing four-bedroom house. Currently the lot is developed. The lot is served by a community water supply. The existing septic system shall be reused. A diverter valve will be installed between the existing tank and the existing field. The surrounding developed lots are served by private water systems and community water system. The private wells are located over 100 feet from the proposed soil absorption system. The community well is located over 200 feet from the proposed upgrade. 1. Conditional COSA. The Owner is in the process of selling this property. The existing system was located and inspected. The system was test for operation and was found that it could not accept the required 150 gallons per bedroom per day (600 gpd). The system is able to absorb approximately 250 gallons per day. There will be no health risk in approving this Conditional COSA as we will be installing the new system in June. 2. Soils. A single test hole was excavated. See the attached soil log. Ground water was monitored for seven days. No ground water was monitored to a depth of 18 feet below ground level after the monitoring period. Bedrock was not encountered in the test hole. It is my opinion, based on the results of the percolation tests and overall soils appearance; an application rate of 1.2 gallons/day/square feet should be used, using a conventional treatment system. 3. Soil Absorption System Design. a. Percolation Rate: b. Application Rate: c. Design Flow: d. Min. Absorption Area: e. Total Depth: f. Effective Depth: g. Width: h. Minimum Length: i. Design Length: j. Surface Yater: There is no surface water within 100 feet of the proposed septic tank and the existing drain field. The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage ditches. 5. Topography: The average topography in the area of the proposed septic system is approximately 2 percent based on the survey information. The proposed upgrade is located in a saddle on the lot. The proposed installation will be greater than 50 feet from the shoulder of the steep slopes. 6. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification that the review is complete and that there are no further comments is received from MoA On -Site Department, the note will be removed and "Released for Construction" drawings will be issued. The proposed installation will not affect the future development of the surrounding or existing lots. There are no wells or septic systems within 100 feet of the proposed septic location. If you have any questions or concerns, please contact me at 272-8218. Sincerely, Steven R. Pannone, P.E. Bragaw Street P. O. Box 196650 Anchorage, Alaska 99519 Subject: Sun Valley Heights S/D, Block 1, Lot 6 Conditional Certificate of Onsite Approval Request Septic System Permit Request Ladies and Gentlemen: P.O. Box 102954 ge, Alaska, 99510 I am writing to request a permit to construct a new septic system be issued for this property and a conditional Certificate of Onsite Approval (COSA) be issued. The system is able to absorb approximately 300 gallons per day. There will be no health risk in approving this Conditional COSA as we will be installing the new system in June. f-sTr6iTS, =-r tco1(LA-P A Nvj-t-rtl Fttbz.zT. 2. Surface Nater: There is no surface water within 100 feet of the proposed septic tank and the existing drain field. When written notification that the review is complete and that there are no further comments is received from MoA On -Site Department, the note will be removed and "Released for Construction' drawings will be issued. The proposed installation will not affect the future development of the surrounding or existing lots. There are no wells or septic systems within 100 feet of the proposed septic location.
