INSPECTION
In our efforts to help you in obtaining a permit to drill a
new well, the Municipality of Anchorage (MOA), Department of
Health and Human Services (DHHS), has advised that your septic
system should be documented. A search of MOA files has

confirmed the existingtistem is undocumented. A site

inspection was performed on the referenced property on March
31, 1995, to determine the type, location, and dimensions of
the septic system.
WELLINSPECTION
&FLOW TEST
During the site inspection, an 8 ft. wide x 8 ft. long x 4 ft.
deep, concrete structure was excavated. The absence of an
outlet line seems to indicate this structure is a holding
