I further verify that based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply and/or wastewater disposal system is (are) in compliance with all applicable Municipal and State codes, ordinances, and regulations in effect at the time of installation. In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MOP, COSA guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test. and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provide any warranty for future performance. nor can we estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed above. This 4 bedroom single family dwelling has not been lived in yet. They are in the final stages of construction and a Certificate of Occupancy will not be issued until early in January. The current closing date is set for January 9, 1987. Since the system was put in over two years ago, your office requires an adequacy test or a Certificate of Occupancy. As per our phone conversation on December 17, 1986, we discussed that due to the close proximity of the issuance of the Certificate of Occupancy and closing date, that it would not be feasible to obtain Health Authority Approval in that amount of time. Therefore, based on our investigation, we request that Health Authority Approval be given for the subject lot without the Certificate of Occupancy. Lewis Environmental Engineer G LOT 5, BLOCK 1 Soils and Ground Water Most of this lot is located on a hill mapped as the gm landform by the U.S.G.S. Test Boring 35, located on top of the knoll near the southern boundary of Lot 6, encountered layered clean sandy gravels and gravelly sands. Test Pit 23A, located on the base of the hill, also encountered similar soils. Thus the landform is probably a glacial alluvium. Glacial alluvium is defined on the Generalized Geologic Map of Anchorage as irregular shaped hills deposited in or near glacial ice consisting chiefly of gravel and sand, moderately well graded and moderately well sorted. Np— round water was observed in Test Hole 35 which was drilled to a 15 foot depth or in the test pits. Ground water seepage was noted from a depth of seven to eight feet in Test Hole 23A. Location of Septic System The preferred septic system drain field location is in the central part of the lot on the hillside or depending on the house location, on the top of the knoll. The septic system should be located as high up on the hill as feasible to avoid any ground water problems. Design Absorption Area A percolation test was performed in Test Boring 35 and the percola- tion rate was three minutes per inch. Based on criteria in the Manual of Septic Tank Practice, the required absorption area is 100 square feet per bedroom. If silty soils are encountered, the required absorption area must be increased.
