The submittal consists of three (3) drawings showing the present improvements on the lot and the adjoining properties, (sheet 1/3), the proposed improvements of the lot, of which only the septic system is subject to this permit application, (sheet 2/3), and a schematic of the septic system, (sheet 3/3). Soil logs and percolation tests of applicable testholes are also enclosed. Based on our review of these documents and our understanding of the design, construction and inspection of the on-site wastewater disposal system, there are several concerns and questions which must be addressed prior to approval by this office. On May 16, 1991 this office issued a permit for the construction of an on-site wastewater disposal system at the subject lot. The permit was issued based on a design prepared and submitted by Flattop Technical Services. On October 21 this office observed that the system had been completely installed. Flattop Technical Services advised that they had not been involved in providing inspection services during the construction of the system and that they knew nothing about the construction of the completed system. On October 23 this office contacted Acreage Systems. Our records indicated that they had notified our office of an anticipated inspection on July 18, 1991. They confirmed that they had completed installation of the system and that Bob Kniefel had served as the inspecting engineer. During that discussion, you reviewed our file which included a copy of the permitted design. You indicated that Kniefel Engineering was not involved with the inspection of the system, that you knew nothing about a design change requiring a lift station, that you had never been on the subject lot and that you would certainly have recalled inspecting the construction of a system that had been designed by Flattop Technical Services. You also indicated that you would contact Acreage Systems to determine why they had indicated that you were the inspecting engineer. On October 25, this office again contacted Kniefel Engineering and again you asserted that Kniefel Engineering was not involved with the inspection of the system. On November 12, this office received and as -built inspection. report submitted by Kniefel Engineering. After disclaiming any knowledge about the construction of the system on two separate occasions, we question the validity and credibility of an as -built inspection report which now asserts that the system was inspected by Kniefel Engineering. Based on our review of available information, this office has legitimate concerns and doubts about whether the system was installed in accordance with Municipal and State regulations and whether the system was ever inspected by an engineer during construction. Our concerns will be allayed if you can provide the following information: 1. Who was the engineer that inspected the construction of the system? AMC 15.65.150 requires that an engineer be present for a minimum of two inspections during system construction. It should be noted that the permitted design required five inspections by an engineer. 2. Who was the engineer that verified the suitability of the sand filter material? Did the material meet specifications? Was the sand filter properly installed? 3. Who was the engineer that authorized and initiated the field design change which required the use of a lift station? Was the design engineer consulted on this change? Why was this office not notified of the field design change? (please reference Engineer Bulletin 90-4) 4. Was the Acreage Systems lift station bedded and backfilled in accordance with the manufacturer's specifications? Has the lift station installation received approval from the municipal electrical inspector? This installation is for a new residence at the above location. The original design included a bed and sand filter design by Flattop Technical Services, During the construction of the house it was found the elevation of the house was such that the sewer outfall from the house was below the design elevation for the proposed bed. An approved lift station (Acreage DHHS #MS 91- 0203) was properly installed with an ABS SEVH4 pump. MOA inspections were called for on approximately 07-18-91. At the time of installation of the lift station the electrical power had not been extended to the pump and no testing could occur. Therefore the submittal of the as -built information was withheld since the inspection was ,not complete. The ground below the lift station was porous gravel which resulted in the use of a 2' sand filter for the bed. Therefore there was no need to install additional gravel on top of the existing gravel already in place. My office was contacted by Mr. Robbie Robinson of your staff regarding the need for the final as -built for building permit purposes. I checked the information and called Mr. Robinson and explained the lift station did not have power and had not been tested. We therefore could not provide the required as -built information. He stated he had personally reviewed the field depths and found that the bed was out of level by approximately one foot. When I suggested this may have been a comparison of the bed to the depth of the monitor tube he stated that was not the case. Subsequent to this conversation, Mr. Robinson visited the site with the installation contractor (Acreage Systems) and did confirm that the bed was level (+-0.01'). ANCHORAGE, ALASKA 99516 This is the required design narrative to accompany the 4 bedroom sewer permit application for Lot 4, Block 2, Southpark S/D, Addn. #1. Copies of the soil test results and design drawings and specifications are enclosed. The measured perc rate for the proposed receiving stratum (see log for test hole #2) is only slightly slower than 1 minute per inch, however the comparable material in test hole #3 is a little cleaner, so we have included a 2' thick sand filter in the design to provide additional protection for the nearby underlying water table. Using an application rate of 0.7 gpd/sq. ft. as prescribed in the wastewater ordinance, this 4 bedroom residence will require 4 x 150 / 0.7 = 857 square feet of bottom area. This is best accomplished by a 20 ` wide by 43 ` long soil absorption bed. We are requesting a waiver of the normally required 15' width so as to lessen the difference in elevation from one end of the bed to the other due to the 2 - 3 % ground slope. This will also better accommodate the equally large proposed replacement bed site shown on the site plan. The downhill end of the proposed bed will be 30+ feet away from a low (2' - 3' high) cut bank, which should not create any daylighting possibility. The bed configuration is designed to stay more than 50 feet away from a higher cutbank in the vicinity of the SW comer of the lot. At the time of our groundwater monitoring during breakup there was no surface drainage within 100 feet of the proposed soil absorption bed or replacement site. This subdivision is served by a community water system, for which the wells are greater than 200 feet from the proposed septic system. The proposed project will have no impact on on wastewater disposal systems on adjoining lots, nor will it have any impact on reserved space/surface and subsurface, or on drainage. The project includes a 1250 gallon septic tank followed by a 20' by 43' soil absorption bed with 1 foot of sewer gravel overlying 2 feet of filter sand. 1.2 Construction shall be as depicted on the approved site plan and design drawings. Minor deviations from these drawings may be allowed or required by the engineer conducting the inspections. All construction procedures and material specifications shall conform with Municipal and State requirements. 1.3 All separation distances shall be in conformance with Municipal requirements, unless specifically waived. 1.4 The contractor shall be responsible to obtain any necessary utility locates, and to work around any buried utilities. 2.0 Septic Tank: 2.1 The septic tank shall be Municipally approved with a capacity of 1250 gallons and two compartments, and shall be set level on undisturbed soil. Each compartment shall be equipped with a watertight manhole cover and a 4" cleanout. If the tank is buried less than 4 feet, it shall be insulated with 2 inches of approved burial type, rigid insulation. 2.2 All pipe connections to the tank shall be equipped with waterproof mechanical couplings. The waste line from the residence to the septic tank shall have a minimum slope of 1/4"' per foot, and the waste line between the tank and the soil absorption system shall have a minimum slope of 1/8" per foot. A cleanout shall be installed within 5 feet of the building foundation, and a double cleanout shall be installed within 5 feet downstream of the septic tank. , 2.3 Special care shall be taken in positioning the tank to achieve the proper slopes for gravity flow from the residence to the tank and from the tank to the soil absorption bed. 3.0 Soil absorption system: 3.1 The soil absorption system shall be constructed by removing the surface soils to expose a gravelly sand stratum at a depth of approximately 5 feet below ground level at the midpoint of the system. 3.2 The bottom of the excavation shall be level. Medium pit run sand may be added as necessary for leveling, plus a minimum of 2 feet of medium pit run sand shall be placed over the entire surface of the soil absorption bed. Sand for leveling or serving as filter material shall comply with Municipal specifications. The top of the filter sand course shall be level. 3.3 A total of 12 inches of sewer gravel shall be placed on the top of the filter sand, with the perforated distribution pipes laid level in the sewer gravel such that the pipe inverts are no less than 6 inches above the top of the filter sand. Sewer gravel shall be 0.5" - 2.5" screened gravel, with less than 3% passing the #200 sieve. 3.4 Monitor tubes and cleanout pipes shall be of 4" diameter and installed in the locations shown on the design drawings. The portion of the monitor tube extending through the sewer gravel shall be perforated. 3.6 Approved filter fabric shall be placed over the entire top surface of the sewer gravel. A minimum of 2 feet of soil cover is to be placed over the filter fabric. If the soil cover thickness is less than 3 feet, two inches of rigid, burial type insulation is to be placed over the entire top surface of the gravel, in addition to the filter fabric. 3.7 The top surface of the cover material shall be raised a minimum of 6 inches higher than the surrounding terrain to allow for subsequent settlement, and shall be graded to smooth contours. Fill slopes shall be no steeper than 3:1. 4.0 Inspections: 4.1 A total of 5 engineering inspections will be required during the course of the project: (1) initial stakeout with the contractor to establish the location of the system and to discuss the plans, specifications and construction procedures, (2) after the native material has been excavated to expose the infiltrative surface to ensure that it is level and at the right elevation, and conforms with the soil test information, (3) after the sand leveling and/or filter material has been placed, (4) after the sewer gravel is in place and the distribution pipes have been laid and connected up to the septic tank, but prior to placement of insulation or filter fabric, and (5) after final backfill and grading is complete. The septic tank requires one inspection after it is set level and the piping connected, but prior to backfill. This inspection may be incorporated with any of the above inspections. 4.2 The installer shall coordinate the timing of the inspections with the engineer sufficiently far in advance to ensure the availability of the engineer. Soils encountered by Test Pits 15 and 28 and Test Boring 5-2 consist of coarse gravels with sand layers and with boulders and cobbles. No ground water was observed in either Test Pit 15 or 28. Ground water was found at 13 feet by Boring 5-2. Water was at seven feet in Boring 3-2A located near the north :Lot line. Location of Septic System The preferred septic system location is on the southern one-half of the lot. Soils in this area may be coarser and the chance of a high ground water table occurring is less probable.
