To Whom it may concern:
This letter is a request for a replacement on-site septic permit. The test hole was completed in August 1,
2014 which revealed poorly graded sands (SP) with water observed at 14 feet but raised to 12 feet after the
7 day monitoring period. The perc rate was faster than 1 minute per inch but the soils have greater than 50
percent sand or sandy gravel. The old 1000 gallon tank will be replaced with a new tank but the old leach
field will be decommissioned due to its encroachment into the neighboring lot. The water table elevation
for the design has been raised 2 feet per the code for August therefore the bottom of the system will be at
elevation -6 feet.
No open water exists within 100 feet of the proposed design. The lot has a slope great than 25 percent but its
uphill of the proposed leach field. This system will not impact any of the neighbors systems due to
community water from AW WU.

Soils and Ground Wat_
Test Pits 5 and 34 were excavated on the edge of the plateau
close to the northeast property line. These pits found granular
soils with no ground water. Borings 4-4 and 3-3 Located on Lots
4 and 3 encountered layered gravelly silty soils and clean sand
and gravel layers. The sand, and gravel layers are quite permeable.
Soil conditions may change significantly over short distances.
Location of septic _system
The preferred septic system location is in the southwestern
portion of the lot.
Design Absorption Area
Percolation tests were performed on Boring 4-4 and 3-3. The
percolation rate for 3-3 was one-half minute per inch and
4--4 was two minutes per inch. Based on these testas and the soil
conditions encountered by Test Pit 5, it is recommended that the
drain field be designed for an absorption area of 140square feet
per bedroom. jSO

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Onsite Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer of
Ede (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

Dear Sirs :
By means of this letter we are requesting issuance of a waiver allowing the existing soil
absorption system on Lot 2, Block 3, Southpark S/D to be re -approved in its existing location 0 feet
from the property line common with Lot 1, Block 3. An as -built survey of the property is enclosed
along with copies of recorded easement documents and other relevant documentation from your files.
When the soil absorption trench was constructed and approved back in 1981 the Municipal
inspector reported in error that it was located 10'+ from the property line. The 1995 11AA application
also reported in error that the system was 10'+ from the property line, but an as -built survey prepared in
conjunction with the sale of the property at that time noted the fact that the monitor tube for the trench
actually encroaches 2.9 feet beyond the property line onto Lot 1. The owners of the two lots entered
into an easement agreement at that time allowing continued use and maintenance of the system and that
easement was officially recorded on 10/27/95 (see printout from Recorder's Office).
A recent adequacy test of the system determined that it is still functioning adequately for the
residence on Lot 2. The system is located approximately 30 feet from the absorption trench presently
serving the residence on Lot 1, so there is no problem related to the proximity of the two systems.
Because all lots in this subdivision are served by a community water system there are few restrictions on
potential replacement systems for Lot 1, so granting the requested waiver should not significantly
compromise the ability to upgrade the system on Lot 1 in the future.

Test Procedure: System was inspected and measured. Tank was found with 6 feet of cover. Liquid depth
was measured to be 4 inches. A single clean-out/monitor tube was found. The monitor tube had a total depth
of 91 inches. A black substance was noticed at the bottom of the clean-out. It was obvious that the lower five
to six feet of the monitor tube was clogged with this black substance. The substance was relatively dry. No
fluid was detected.
Water was added to the system at a rate of 3 g.p.m. The water levels in the tank and monitor tube was
monitored. Six hundred gallons of water was added over a three and one half hour period. The level in the
monitor tube rose from no fluid to 27.5 inches deep, Approximately two inches above the lateral invert.. The
level stabilized at that depth. Fluid levels were monitored in the tank. The fluid depth remained a constant four
inched in the tank.
The system was allowed to absorb the liquid fora period of ten minutes after the 600 gallons of water was
added. A six inch drop in fluid level was noted during this period. Twenty four hours later the water was
monitored again. There was no indication of water in the stand pipe.
TESTS RESULTS: This system meets the code requirements of the Municipality of Anchorage.
This system is 14 years old and appears to operating in the last 15% of its usefulness. The operational life of
all septic systems depend on the local soil condition, ground water levels that may fluctuate during the year,
and the water usage of the family being served by the system. These conditions are outside the control of the
evaluator of this system. We can therefore not give any estimate of how lorft�tem well continue to meet

the operational requirements of the Municipality and State.
