Our records show that the permit referenced above has not been properly closed and is expired. This means the
system involved is not approved to operate as required by Anchorage Municipal Code (AMC). The use of an
unapproved well is subject to fines under AMC 14.60.030. Additionally, an unapproved well can cause a costly
delay when selling a home.
In order to close this permit, a Pump Install Log is required to be sent to the Onsite Water and Wastewater
office for review and approval.
If the requested information is not provided within 30 days, a Notice of Violation (NOV) of municipal code
may be issued. A NOV can lead to a citation, fines, etc. Please contact this office if you are unable to provide
the requested information within 30 days, or if you have questions. We will work with you to close the
outstanding permit.
