2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing
Special Provisions:
1. Please confirm if the 2001 septic system was installed. Note on the IR.
2. Locate the water line to ensure that the required 10' separation will be met.
3. Locate the edge of the crib to ensure that the tank meets the required 5' separation from it.

RE: SEPTIC TANK UPGRADE PERMIT
LEGAL: SUNNY VALLEY LOT 10 REM
The owner has requested that we obtain a septic permit to upgrade the existing aged steel septic
tank on the above referenced lot. For current functionality and future flexibility, we propose to
install a 1500-gallon HDPE tank per the attached design to serve the existing 4-bedroom
residence. No groundwater was noted in the MOA on-site file, but if groundwater is encountered
during installation an epoxy coated steel septic tank may be required.
The lot and area are served by private water and all wells are 100’+ away from the proposed
septic tank location. The existing water line must be staked prior to construction and be 10’+
away from the septic service lines and tank. The 2001 permitted field appears to not have been
installed and will be reverified and confirmed at construction. The design will not impact any of
the neighboring properties. Please contact us if you have any questions.

Dear Turner Pippen:
An On-Site Water/Wastewater Permit, number SW010285, issued by this office for a single-family
system, will expire on August 1, 2002. This permit was valid for 365 calendar days.
If this was a well permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as-built inspection report must be sent to
this office for review, approval and documentation. This as-built inspection report must be signed by
the licensed Professional Engineer who inspected the installation of the system. As-built inspection
reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A new permit for a second year may
be issued for a fee of $100.00 if the renewal application is received on or before the expiration date of
the original permit.
When applying for a new permit, the fees are: $400.00 for a wastewater permit and $150.00 for a well
permit.

It is requested that you issue a permit to upgrade a septic system to serve the existing
four bedroom dwelling on the referenced property.
A test hole was excavated and a percolation test was performed. The approximate location
of the test hole is located on the attached site plan. At the time of excavation on 6/17/01
the test hole was found to be dry. After seven days of ground water monitoring the test
hole was still dry.
We do not anticipate any adverse effects on neighboring wells, septic systems, reserve
areas or drainage patterns by the installation of the proposed septic system. The
construction of this system will not prevent any future development on any of the adjacent
properties.

This is to advise you that I am the owner of Sunny Valley subdivision lot 10, platt 631,
located at mile 4 Eagle River, Rd.
This is also to advise you that I have installed two previous septic systems in the City of
Anchorage and both systems passed inspection by the City of Anchorage inspectors at the
time. I fully understand the operation ora septic tank/Leech field system, the types of
materials used, and the installation and inspection procedures.
I further state that I am proficient with the use of Dozers (I own and operate one), back
hoes, front end loaders, and excavators. I have rented and used each of these types of
heavy equipment on several different occasions.
I will be installing a new septic tank/Leech field, on this lot to replace one that I had
installed 24 years ago. The design work for the new system Was performed by S&S
Engineering of Eagle River, and the inspections will also be done by their Company.
