Is the following items resolved? 1. Installed baffle at the tank inlet 2. Sand and recoat all places on the tank were welding was done, bare metal present, or tank damage. 3. Cut air gap in the interior baffle 4. There were welds on the interior baffle that daylight could be seen through. Is the well now continuous? 9/4/2008 Page 2 of 2 The MOA also wanted a brief description from you in writing as to how the metal was prepared and applied. If you send the description to me via email, I will forward it to the MOA. Once all of the aforementioned items are done it is my assumption that you have done everything the MOA required in order to install the tank. Box 196650, Anchorage, Ak 99519-6650 (907) 343-7904 Ref: Proposed Septic System for USS #3202; Lot 17, To whom it may concern: There was a previous permit issued for this property on 7/13/05 for a well and septic design. Per the homeowner request, we re -designed it from an advanced treatment system to a conventional system and reduced it from a 5 bedroom to a 3 bedroom system. The existing 3 bedroom house is served by a private well and will be served by a private septic system. We are proposing to install a 1250 gallon S.T.E.P. tank and dual 5 -wide trench type drainfields. Two test holes were excavated on the property by Tobben Spurkland P.E. The drainfield will be designed around the 30 foot radius of test hole #2. Comments regarding the design are summarized as follows: 1. SOILS: See the attached Togs which show the soil classifications and the percolation test results. Groundwater monitoring was done on 5/16/08 and found that water was at 4 feet in test hole #2. 2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications. 3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic system. 4. TOPOGRAPIIY: As can be seen on the attached design drawing the average topography below the drainfield is a 10 percent slope running approximately northeast to southwest. We are unaware . f any adverse impacts this installation would have on adjacent wells or septic systems. If you have any •t testions, please contact us at 337-6179. Thank you for your assistance. fr Pres E., M.S. NO : , trached is a site plan drawing. a design drawing, one soil log, which are all part of the design package for this septic system. (Contact C. The scope of this project is investigation for suitability of an on-site sewerage system. Included in this transmittal are: Test Hole Log Table A Explanatory Information Sheets 1-3 The exploration was conducted using a continuous flight solid stem auger. The rig is owned and operated by Denali Drilling, Inc. Drilling was supervised, the test holes logged and percolation test performed by Ms. Mike Hewitt, Technician with Alaska Testlab. The log of the test hole is included as Table A of this report. In interpreting the log it would be helpful to utilize the explanatory information contained in Sheets 1 to 3 of this report. When drilling was completed a 3/4" slotted PVC pipe was inserted in the hole to aid in determining the free water level. For the percolation test, the test .hole was filled with water and left overnight to saturate. On returning the next day, the hole was refilled with water and the drop in the water level carefully monitored over the next 60 minutes. This procedure is not a standardized percolation test, however, we understand that the Municipality of Anchorage, Department of Public Health and Environmental Protection prefers tests performed in this manner to evaluate a site for a proposed on-site sewerage system. Bonnie Wilson ,Th February 22, 15 Page 2 Using the above test, the observed minimum percolation rate was 100.0 minutes per inch. Overnight the water level only dropped 2". The water table was observed during drilling to be at 11'. It should be noted though, that the free water level normally fluctuates seasonally and with precipitation. We hope this report meets your present needs. If we can be of further service, please feel free to contact us. Sincerely, ALASKA TESTLAB R.
