Wastewater Disposal Regulations ( 18AAC72 ) and Drinking Water Regulations ( 18AAC80 ).
3. The engineer must notify DHHS at least 2 hours prior to each inspection. Provide notification by calling
(907) 343-4744 ( 24 hours ). ( Not required for a Water Supply Permit only ).
4. From October 15 to April t5, a subsurface soil absorption system under construction during freezing weather
must be either: A. Open and closed on the same day.
B. Covered, sealed, and heated to prevent freezing.

AMENDED.
2 INSTALL NEW 1250 GALLON, 2 COMPARTMENT STEEL SEPTIC TANK. REMOVE & DISPOSE OF EXISTING TANK IN
ACCORDANCE WITH TITLE 15.65 REGULATIONS.
3 CONTRACTOR TO LOCATE ALL UNDERGROUND UTILITIES PRIOR TO COMMENCING WITH CONSTRUCTION.
4 CONTRACTOR TO NOTIFY THE ENGINEER AND OHHS 24 HOURS PRIOR TO COMMENCING WITH CONSTRUCTION.
MINIMUM SOIL COVER SHALL BE 4 FEET OVER THE SEPTIC TANK. ONE INCH OF INSULATION MAY BE SUBSTITUTED
FOR 1 FOOT OF' SOIL COVER BUT THERE MUST BE A MINIMUM OF 2' OF COVER OVER THE INSULATION. ALL
INSULATION BOARD SHALL BE (DOW H1-55 INSULBOARD OR ENGINEER APPROVED EQUAL).
MATERIAL USED AS FILL SHALL BE CLEAN AND FREE OF ORGANICS, TRASH AND CONSTRUCTION DEBRIS.
GRADE ALL FILL MATERIAL IN SUCH A MANNER THAT PONDING AT OR NEAR THE SEPTIC TANK DOES NOT OCCUR.
THAN SHOWN TO ALLOW FOR SETTLEMENT, SEED SURFACE WITH GRASS MIX AFTER COMPLETING INSTALLATION.
LOCATION OF EXISTING LEACH FIELD IS NOT DOCUMENTED. LOCATION OF FIELD CLEANOUT IS SHOWN PER AS-BUILT
