To whom it may concern:
A septic tank was installed in 2011 was placed in the same location as the old tank, 2.0 feet away from the
east property line. We are requesting that the existing waiver (WR49900020) will be amended to reflect
the new installation. Justifications for the waiver request are blow;
• There are no septic systems with in 50 feet of the septic tank.
• The septic tank was installed in the same hole as the old septic tank which encroached for 12 years
with no adverse impact.
• There is an existing waiver to 2 feet (WR#990020) from the old septic tank.

Ref: Proposed Septic Upgrade for Sky Harbor Estates #1 Lot 4
To whom it may concern:
The existing 3 bedroom house is served by a private well and septic system. The septic system is in a state
of failure and needs to be upgraded. We are proposing to inspect the existing 1000 gallon septic tank for
structural integrity. If it is structurally unsound we will replace it with a new 1000 gallon septic tank. We
are also proposing to install the reserve drainfield at the location identified on the 1999 design. Per the
1999 design the reserve site will will use the insitu soils as a sand filter. Two test holes were previously
excavated on the property. The drainfield will be designed around the 30 foot radius of test hole 92.
Comments regarding the design are summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic system.
4. TOPOGRAPHY: As can be seen on the attached design drawing the topography around the
drainfield is relatively flat. In short, there are no slope concerns.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems.

Ref: Well and Septic Design for Lot 4, Sky Harbor Estates Subdivision #1
To whom it may concern:
The proposed 3 bedroom house will be served by a private septic system and a private well. Two
test holes were excavated on the property. We are proposing to have the well drilled in the
northwest corner of the referenced property and installing a septic system within the 30 feet radii
of the test holes. Comments regarding the proposed design are summarized as follows:
1. SOILS: Attached are logs which shows the soil profile, and the percolation test results. In
test hole 41, the soils below the organic layers is a ML material (with a 6 inch SW lense at 3.0 to
3.5 feet) to a depth of 5.0 feet and than transitions to a SP material to a depth of 15 feet (bottom
of test hole). In test hole #2, the soils below the organic/fill layers is a SM/ML material to a depth
of 15 feet (bottom of test hole). No groundwater was encountered during the excavation of the
test holes. A percolation test for TH#1 was performed between the depth of 6.0 feet to 6.5 feet
which had a percolation rate of <1 minute/inch. A percolation test for TH42 was performed
between the depth of 5.0 feet to 6.0 feet which had a percolation rate of <1 minute/inch. It is our
opinion that due to the high percentage of sand, the insitu soils will serve as a sand filter. A grab
sample can be provided if deemed necessary.
2. TRENCH DESIGN:
a. Percolation Rate: <1 minutes/inch
b. Allowable Application Rate: 1.2 gallons/day/ft2
c. Number of Bedrooms: 3
d. Design Flow: 450 gallons per day
e. Minimum Absorption Area: 375 ft1
f. Total Depth: 9 feet (max.)
g. Effective Depth: 4 feet
h. Width: 5 feet minimum
i. Reduction Factor: 0.50
i. Minimum Length: 40 feet total length (2 @ 20 feet long each)
j Effective absorption area = 400 It
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed
upgrade.
4. TOPOGRAPHY: The topography of this property runs approximately northwest/west to
southeast/east at a slope of 5 to 10 percent. In the area of the proposed drainfields it is mostly
flat, in short, there are no slope concerns.
5. LOT LINE WAIVER REQUEST: We request that your department issue a 2 foot lot line
waiver from the east property line to the proposed and alternate drainfields. We are unaware of
any adverse impacts to the east neighboring property with the granting of this waiver.

In conducting this evaluation, GEG, LtD_ attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.

Ref: Request for COSA with a Portion of 2011 Drainfield 'Under Driveway:
To whom it may concern:
As can be seen on the attached as -built survey done by Lantech, a portion of the 2011 drainfield
(approximately 3 feet) is located underneath the driveway. We are requesting that your department will
issue the COSA based upon the following justifications;
The buyers of the property should be notified of the issue and be advised to avoid driving over the
portion of the driveway & the increased possibility of the drainfield freezing during winter months.
The drainfield has now gone through 2 winters with out any reported freezing.
The drainfield has a minimum of 2 feet of soil cover and 2 inches of blue board insulation on top. In
short, we do not anticipate a future freeze-up.
