Septic tank replacement
Dear On-Site Services Engineer:
The septic tank on the above lot is 40 years old, and the owner has decided to replace it. The existing
home is a 4-bedroom. We are submitting this permit application for the placement of a new septic tank.
The attached site plan identifies the location of the existing home, well and septic system, as well as
the location of the new septic tank. The drainage pattern in general will not be changed by the
construction.
The existing tank will be pumped dry and decommissioned per code. It is a 1500-gallon tank and
currently sits within 10’ of the house foundation, underneath the exterior deck. The deck will be
completely demolished and not rebuilt. The homeowner would like to replace the tank with a 1250-
gallon one, since the house is currently a 4-bedroom home. The new tank will then be placed outside
the 100’ well radius and 10’ away from the foundation of the home.
After the new tank, we will install a double-cleanout and connect into the existing absorption bed. A
double cleanout will also be placed before the tank, or a foundation cleanout if that can be installed.

conducted on this property with the following findings.
Total depth of well is 160 ft deep
Static water level 56 ft
Well produced in excess of 10 g.p.m for 4 hrs
Well had temporary electrical line above grade during time of testing. Client understands that #10 u/f AWG will need to
be buried upon completion of current excavation and grading of road access to property.

Dear On-Site Services Engineer:
The septic tank on the above lot has failed, so we have submitted a permit for a tank replacement. This
permit has been approved.
However, the owner and buyer discovered this during the sale of the home. The seller is unable to
extend the purchase period of the home for some financial reasons. We request that a conditional COSA
be issued, so that the new owners can take possession of the home and have the work completed after.
They are in agreement with this arrangement.
We have obtained three bids from different contractors, and 150% of the high bid is being placed in
escrow. The work will be completed within the next two weeks, so there is no reason to delay the
COSA in order to verify the work first.
The work to replace the system will happen as soon as possible. Currently, there is no effluent
daylighting or coming up anywhere. There is no threat to health and safety in issuing this conditional
COSA.
