The test holes were advanced by means of a truck- mounted auger fo the depths shown on the attached data sheets. The soils encountered were logged in accordance with the Visual Unified Soil Classification System. The significant soils were silty gravels and gravelly silts. These soils are frost-susceptible. It is recommended fhaf any footings placed on these soils be drained and insulated. One satisfactory method is fo provide 6 feet of cover on fha exposed side of fha footing. The performance of the percolation fast in accordance with the method prescribed in the FHA publication, Minimum Property Standards, was not fully practicable due to the soil conditions which caused excessive sloughing of the test holes and also hy a period of relatively rainy weather with a consequent temporary rise in the water fable. Based on the soils and observations of the area, it is my opinion fhaf fha soil in the area of les} hole 2 would have a percolation rate in fha range of I inch per 30 fo 60 minutes. A waste disposal system at this site should be designed for greater than minimum retention period. Federal }lousing Administration or Veterans Administration will furnish the form when applicable, des- ignating the Public Health Authority. Percolation tests and soil borings shall be made and tabulated in Tables I and II, Part II below, by a licensed Engineer or qualified Sanitarian. This report in duplicate, accompanied by the required top- ographic map, shall then be submitted for review and analysis to the following Health Department: (Name and Address) INSTRUCTIONS TO ENGINEER OR SANITARIAN: 1. An adequate number of tests shall be made (one per acre, or if soil conditions indicate, a greater number will be required) to show clearly the absorptive ability of the soil throughout the tract. (Use Table I) 2. Each test hole shall be located by a key number on a topo- graphic map of the tract. 3. Soil borings shall be made (one every 5 acres, or if subsoil conditions indicate a greater number will be required) to show clearly the type of soil existing beneath the absorption area. l~orings should extend to a point at least 6 feet below the finish grade of proposed absorption trenches. (Use Table II) PROCEDURE TO FOLLOW IN MAKING REQUIRED PERCOLATION TESTS: 1. Dig or bore the holes with horizontal dimensions of from 4 to 12 inches and vertical sides to the depth of the bottom of the proposed absorption device. Holes can be bored with 4 inch di- ameter post-bole type auger. 2. Roughen or scratch the bottom and sides of the holes to pro- vide a natural surface. Remove all loose materials from the hole. Place about 2 inches of coarse sand or fine gravel in the hole to prevent bottom scouring. 3. Fill thehole with clear waterto n minimum depth of 12inches over the gravel. By refilling, if necessary, or by supplying a surplus reservoir of water (automatic siphon), keep water inhole for at least four hours, and preferably overnight. In sandy soils ie GW, GP, SW, or SP classified according to the " Unified Soils Classification System", the above maturation procedure is not necessary and the test can be made after the water from one fill- ing has seeped away. 4. Percolation rate measurements should be made on the day fol- lowing the saturation process, except in sandy soils. S. If water remains in the test hole after overnight saturation, ad- just the depth to 6 inches over the gravel. From afixed reference point, measure the drop in water level at approximately 30-minute intervals over a 4 hour period. The drop which occurs during the final 30-minute period is used to calculate the percolation rate. Note: The engineer should determine if the water in the test hole is due to a high ground water condition or the permeability of the soil. Report ground water conditions on reverse of Form and soil types in Table Z 6. If no water remains in the hole after overnight saturation, add clear water to a depth of about six inches over the gravel. From a fixed reference point, measure the height of the water surface at approximately 30-minnte intervals over a 4 hour period, refilling the hole to a depth of 6 inches when the percolation rate indicates the bole will run dry before the next reading is made. The drop which occurs during the final 30-minute period is used to caicmlate the percolation rate. Not.e: If a hole must be refilled to obtain a final 30-minute reading, determine from the previous reading the water level drop during that interval. Add water until the level above the bottom equals this figure plus one half inch. Continue, measure drop during the final 30-minute period. 7. In sandy soils, or other soils in which the first six inche~ of water seeps away in less than 30 minutes, after the overnight sat- uration period, the time interval between measurements can be taken as 10 minutes and the test run over a period of one hour. The asbuilt survey dated September 19, 1995, submitted with the request, identifies the encroachment. Also shown is a 6' wood fence which encroaches into the same easement. AWWU 48' water mainline is located within the East 88th Avenue right-of-way and is not considered available to the referenced lot. A mainline extension agreement will be required to provide public water. AWWU sanitary sewer mainlines are located west of the lot within the North Circle right-of-way and north within the East 88th Avenue right-of- way. AWWU has no objection; however petitioner should provide DPW with verification of septic system locations on adjacent lots or DHHS well permit for Block 6, Lot 3A existing well. NOW, THEREFORE, for and in consideration of their mutual agreements and other good and valuable consideration, the receipt of which is hereby acknowledged, First Party, for itself, its successors and assigns, waives its objections to Second Party's encroachment on the said easement for the purpose mentioned, .subject to the following conditions: 1. Second Party will hold First Party harmless from liability for any injury or damage to any person or property which may result from such encroachment. 2. The said encroachment will be without prejudice to First Party's full enjoyment of any and ali_ rights it may have in and to such easement. 3. Any facilities or property of First Party which are damaged or destroyed as a result of such encroachment will be repaired or replaced at no cost to First Party. 4. Second Party will comply with all applicable safety codes and regulations. 5. Any expenses which First Party may incur in the relocation or modification of its facilities to accommodate the said encroachment will be paid by Second Party. 6. A sign, warning of the dangers of working near the overhead lines, will be installed adjacent to the well casing by the First Party. By its execution of this Agreement, Second Party agrees, for itself, its successors, and assigns, that its encroachment upon the named easement as hereinabove mentioned will be subject to the foregoing conditions.
