Ref: Proposed Septic System for Lots 5; Stelioes Subdivision
ADVANTEXT'I TREATMENT SYSTEM
To whom it may concern:
1. GENERAL: The proposed 4 bedroom home will be served by a private well and septic
system. An exploratory well permit was issued on June 22, 2000 (permit #SW000185). The well
has been drilled in the area shown on the attached site plan and the well log is also attached. A
total of four test holes were excavated on the property. We are proposing to design the septic
system around the 30 foot radii of TII#1 & TH#2. Due to the shallow bedrock, the limited
amount of receiving soils, and the limited area for septic system due to building setback
requirements, we are proposing to use an Advantex treatment system and a 5 -wide trench type
drainfield. Comments regarding the design are summarized as follows.
2. SOILS: See the attached logs, which shows the soil classifications, groundwater monitoring,
and the percolation test results.
3. DRAINFIELD APPLICATION RATE: The intent is to install an Advantex Treatment
System that will allow the use of a small drainfield in the area around the 30 foot radii of the
TH#1 & TH#2. A percolation test was performed in each of these test holes between the depths
of 3.0 to 3.5 feet and found the percolation rates to be 25.3 and 26.5 minute/inch. Given the
performance of he Advantex system, we anticipate that they will receive class III approval.
Regardless, we are going to conservatively assume an application rate of 2.0 GPD/FT . Given
the level of treatment provided, we are requesting a 4 feet separation distance to bedrock.
4. TRENCH DESIGN:
a. Percolation Rate: 25.3 & 26.5 minutes/inch
b. Allowable Application Rate: 2.0 gallons/day/ft2
c. Number of Bedrooms: 4
d. Design Flow: 600 gallons per day

5.ORENCO PACKAGE SYSTEM: The STEP tank with the Advantex Treatment System will
be manufactured by Anchorage Tank &. Welding to meet the latest design criteria established by
Orenco Systems, Inc. The point of contact at Anchorage Tank is Lowell McNutt. As with their
standard STEP tank, it is equipped with a high water alarm per M.O.A requirements.
6. SURFACE NATER: There is no surface water within 100 feet of the proposed septic
system upgrade.
7. TOPOGRAPHY: As can be seen on the attached topography site plan, the average
topography where the proposed drainfield is to be installed is a 5 to 10 percent slope running
from approximately northeast to southwest. In short, there are no slope concerns.
8. CLOSING: I am open to any suggestions from your department that would be an
improvement to the proposed system. I am unaware of any negative impacts that this installation
would impose on adjacent wells, or septic systems. If you have any questions, please call us at

verify that the engineer has inspected all effluent and air pumps, timers, and alarms, and that any
deficiencies have been repaired and that the system is functioning as designed.

To whom it may concern:
A septic permit was issued on this property on May 14, 2002 (permit #SW020110). We
originally requested that the required 6 feet separation distance to bedrock from the bottom of the
drainfield be waived to 4 feet given the use of the Advantex System. It is our understanding that
the separation distance to bedrock has been questioned and that your department will only issue
the 4 feet separation distance to bedrock on a case by case basis. The following is justification
for the requested waiver:
It is our understanding that the Advantex system meets and has received a class III approval. The
only other system that has met this requirement is a Lined Intermittent Sand Filter (ISF) system
with a drainfield. Drainfields that use Lined ISF systems have been approved by your department
to be installed 4 feet from bedrock. Both the Lined ISF and Advantex systems have had sampling
performed on the effluent being discharged to the drainfeids. The sampling performed on each
system included testing for nitrates. The nitrate levels from the effluent from the Advantex
system were found to be lower than the effluent from the Lined ISF. Given that nitrate levels is
the main concern regarding separation to bedrock, it is our opinion that the Advantex system
meets the same requirement as the Lined ISF. Therefore, we request that the 6 feet separation
from the bottom of the proposed drainfield to bedrock be waived to 4 feet.

SUBJ: Waiver Request of the Absorption Field Bottom to Bedrock
The subject lot has bedrock located at approximately 9 feet from
ground surface. The engineer Michael N. Ancerson, PE requests a waiver
of the required 6 feet separation of the bottom of the drainfield to bedrock
down to 4 feet of separation. See attached waiver request dated October
14, 2002.
The following points were discussed by Jim Cross, PE, Program
Manager of the On -Site Water & Wastewater Program and Keven Kleweno,
PE, of the Alaska Department of Environmental Conservation and myself:
1. Water wells in the area appear to be relatively deep at
approximately 400 to 500 feet f cnsrD ro losasr+-
2. Soils the absorption field will percolate into are rated as ML with a
percolation rate of 20 + minutes/inch
3. Water sample results of the well water indicate no bacteria and no
nitrates detectable
4. The slope of the bedrock appears to be downward from the
location of the well on the subject lot to the proposed absorption field .

To Whom it may concern:
This is a request for a waiver of 4 feet to bedrock instead of 6 feet for the above referenced property.
A new AdvanTex System has been permitted (SW020110) for this lot and the tank has been installed.
The following reasons are the justification for the waiver to be granted.
A. The large lot size ( R-6, 1 1/4 acre minimum), smaller lots existing to the east, but all the other
surrounding lots are large R-6 minimum.
B. The existing soils are tight silts, 25 minutes per inch with bedrock at 9 feet per the soils log for
this lot. The neighboring lots also have slow silts.
C. A water sample test has been included from the neighboring lot to the east showing low nitrate
analysis, see copy attached.
D. The wells in the area are deep, this lot has a well over 450 feet and the neighbor to the east is
507 feet, lot 13 Bonnie View Subdivision see plan.
This granting of this waiver will not affect the future development of the neighboring properties or
endanger the water quality of the area.

During a recent COSA on-site inspection and test of the potable water
supply well on Block , Lot 5 of Stelioes subdivision, the well's
productivity was determined to be .75 gallons per minute. The minimum
well productivity required by this Department (AMC 15.55) for a 4 -bedroom
residence is .41 gallons per minute. Although the subject well currently
exceeds this minimum requirement, all parties concerned are advised that the
production capacity of the well may fluctuate. Restriction of non-critical
water uses such as washing cars and watering lawns and gardens may be
required.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

AND
THIS MAINTENANCE AND REPAIR AGREEMENT made and entered into as of this

herein the "OWNER," and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Maintenance and Repair Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. Municipality grants permission to
Owner to utilize and operate an Advanced Wastewater Treatment System
(AWWTS), described as
Advantex Wastewater Treatment System
located at (legal description).
Lot 5, Stelioes Subdivision
2. Definitions.
Alteration. Any change to the design or function of an AWWTS that
includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design.
Certificate of On -Site Systems Approval (COSA). An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with Anchorage
Municipal Code (hereinafter, "AMC") 15.65. These approvals certify that
service and maintenance provider approved by the municipality and the
manufacturer of the AWWTS for the entire term of the AWWTS. In addition, it
shall be the responsibility of the Owner during the term of this Maintenance and
Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay
for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement
costs, and (5) inspection costs.
B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules
and orders for the AWWTS.
C. Upon request by the Municipality, the owner agrees to provide the Municipality a
written schedule of routine maintenance and repairs which have been performed
on the system. When a record of maintenance is documented and maintained by
the system vendor, the owner agrees to allow the Municipality access to this
information.
D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS
may be assessed in accordance with AMC 14.60 for improper discharge.
E. Owner agrees that only maintenance and repair personnel approved by the
Municipality will inspect and make any necessary maintenance, repairs or
permitted alterations to the system.
F. Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
G. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On -Site Systems Approval.
H. Owner agrees that the AWWTS installation and maintenance requirements as
provided by the AW WTS vendor/installer and approved by the Municipality are
the governing guidelines for the construction, maintenance and repair of the
Owner's AW WTS.
6. Nonwaiver. The failure of either party at any time to enforce a provision of this
Maintenance and Repair Agreement shall in no way constitute a waiver of the
provisions, nor in any way affect the validity of the Maintenance and Repair
Agreement or any part hereof, or the right of such party thereafter to enforce each and
every provision hereof.
7. Amendment.
A. This Maintenance and Repair Agreement shall only be amended, modified or
changed by a writing, executed by authorized representatives of the parties, with
the same formality that this Maintenance and Repair Agreement was executed
with, and such writing shall be attached to this Maintenance and Repair
Agreement as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this agreement, the only authorized representatives of the parties
are:

representative or unauthorized means shall be void.
&. Jurisdiction: Choice of Law. Any civil action arising from this Maintenance
and Repair Agreement shall be brought in the Superior Court for the Third Judicial
District of the State of Alaska at Anchorage. The laws of the State of Alaska shall
govern the rights and obligations of the parties under this Maintenance and Repair
Agreement.
9. Severability. Any provisions of this Maintenance and Repair Agreement decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining
provisions of the Maintenance and Repair Agreement.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority Approval are
valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued with
new water sample results. (Certificates may be reissued for a period of up to one year with valid water samples.)
Certificates are valid for one year for properties served by Class A or B wells or a public water system. The
Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

Health Authority Approval # 030197
During a recent Health Authority Approval on-site inspection and test of the
potable water supply well on Block , Lot 5 of Stelioes subdivision, the
well's productivity was determined to be 0.8 gallons per minute. The
minimum well productivity required by this Department (AMC 15.55) for a
4 -bedroom residence is 0.42 gallons per minute. Although the subject well
currently exceeds this minimum requirement, all parties concerned are
advised that the production capacity of the well may fluctuate. Restriction
of non-critical water uses such as washing cars and watering lawns and
gardens may be required.
This advisory must be attached to all copies of the subject Health Authority
Approval.
