Upgrade Septic System Permit Request Narrative
osP 211388
This is a design narrative for a permit to upgrade the septic system at this property. The proposed
system will have capacity for a 3-bedroom house. The existing tank and field are greater than 40 years
old. The existing septic tank will be removed and disposed of. The system will utilize a replacement
1,000-gal septic tank and new deep trench.
t. Soils. One test hole was performed in the vicinity of this system by Eklutna Engineering in
August 2021., and groundwater was monitored for at least seven days. Ground water was not
observed to a depth of 17' below the surface in the test hole monitor tube in August of 2O2t.
lmpermeable layers were not encountered in the test hole to L7'. The soil percolated at < 1
min/inch. Gradation analysis shows that this soil is poorly graded sand. A deep trench will be
constructed. Maximum total excavation will not exceed 11'.
2, Soil Absorption System Design. See Sheet 1 of the design package
3. Wells. This lot and the surrounding lots are served by private water wells. MOA has
documentation for these wells. There are no wells within 100' of the septic system.
Neighboring Wastewater systems. lmmediate neighboring septic systems are all +30' distant.
Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain
field. The proposed drain field upgrade will maintain at least 100 feet from all surface water and
drainage ditches.
5. Topography: The area where this trench will be installed has a slope of less than L%.
The proposed installation will not affect the future development of this or the surrounding lots.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 5 by an Independent professional
civil engineer registered in the State of Alaska. Certificates o! Health Authority Approval are required for the
transfer of title (except between spouses) for properties served by a single family on-site wastewater disposal
and/or water supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health
Authority Approval are valid for 90 days from the date of issue for properties served by a private or Class C well
and may be reissued with new water sample results less than 30 days old. (Certificates may be reissued for a
period of up to one year with valid water samples.) Certificates are valid for one year for propedies served by
Class A or B wells or a public water system. The Municipality of Anchorage is not responsible for errors or
omissions in the professional engineer's work.
4. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I vedty that my
investigation, based on procedures outlined, in .the Health Authority Approval Guidelines for this
application, shows that the on-site water supply and/or wastewater disposal system is(are) safe, functional
and adequate for the number of bedrooms and type of structure indicated herein. I further verity that
based on the information obtained from the Municipality of Anchorage files and from my Investigation and
Inspection, the on-site water supply and/or wastewater disposal system is(are) in compliance with all
applicable Municipal and State codes, ordinances, and regulations in effect at the time of installation.

Subject: Undocumented Well & Septic - Fire Lake, Lot 3, Tract K-1
Gentlemen:
The well and septic system of the above subject property were undocumented. The 2-
bedroom house was built in 1960 and the well and septic conformed to the applicable
separation requirements of that year.
On April 19, 2001 we excavated one testhole within 20' of the existing septic field for
verification of ground water and bedrock or impermeable barrier. Water was not
encountered during the excavation or monitoring. Attached is the soils log of the
. testhole. The septic tank is approximately 1,000. gallons and ou. tfalls into a crib~with an
approximate depth of 5 feet. The septic field/crib is operating m the top 1.5 feet of the
absorption system.
Due to the high levels of nitrates, the existing well has been properly decommissioned
in accordance with AMC 15.55.060 requirements. A 1,000-gallon water storage system
has been installed. We do not expect there to be any adverse effect on adjacent lots
from the subject property's decommissioned well.
