An On -Site Water/Wastewater Permit, number SW000313, issued by this office for a single-family
system, will expire on August 21, 2001. This permit was valid for 365 calendar days.
If this was a well permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be sent to
this office for review, approval and documentation. This as -built inspection report must be signed by
the licensed Professional Engineer who inspected the installation of the system. As -built inspection
reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A new permit may be issued free of
charge for a second year if the application for the renewal is received on or before the date of expiration
of the original permit.
When applying for a new permit, the fees are: $320.00 for a wastewater permit and $120.00 for a well
permit.

On behalf of our client, Claudia Montgomery, we are requesting a waiver of separation distance
between the lotline and the subsurface leachfreld on the subject lot. The system was constructed
5' from the actual lot line. We are making this request as we turn in the asbuilt for the system.
This will not affect the reserve area, and will in fact allow more area for its placement. The soils
in the area are excellent, and the topography is not a factor in limiting area for adjacent lots. The
adjacent lot is a large tract of unsubdivided land, Camp Gorsuch of the Boy Scouts.

Dear Mr. Cross:
The proposed well and septic system will have very limited impact on adjacent properties for the
following reasons:
The surrounding lots are large, allowing sufficient room for septic sites.
2. Immediate neighboring septic systems are all +30' distance.
3. Reserve space is adequate, due to absorption capacity.
4. Drainage will not be affected and is not a major consideration in our design.

SPECIFICATIONS FOR ON-SITE SEPTIC SYSTEM
LEGAL: Steele Estates Lot 2

A. GENERAL
1. The well and septic plan are for a single family residence only.
2. The drawing and or site plan shall be a part of this specification.
3. All materials and workmanship shall meet the Anchorage Department of Health and State
Department of Environmental Conservation requirements.
4. All soil tests are advisory to the design and are to be verified or modified in the field by the engineer.
5. All excavations and depths are advisory and are to be verified in the field by the contractor to meet
Municipality of Anchorage, Department of Environmental Conservation requirements.
6. It is the responsibility of the owner to obtain all necessary permits or easements and to locate any
adjacent multi -family wells.
7. The excavation is to be exactly in the area shown on the site plan, any deviation requires engineer
approval.
8. It is always recommended that a surveyor locate the nearest lot line position and the location of any
easements.
9. Any remaining open test hole excavations shall be filled.
B. SEPTIC TANK
1. Septic tank shall have a minimum capacity of 1000 gallons and be of MOA approved design.
C. TRENCH
1. The trench is to follow the natural land contour to maintain uniform total depth of the trench bottom.
2. The bottom of the trench shall be level, plus or minus 1.5".
3. The total depth of the trench excavation is not to exceed 4' at any point.
4. The effluent line within the trench shall be laid level within 0.03'.
5. The trench gravel is to be covered with typar fabric material.
6. Soil or combination of soil and extruded board insulation to a depth of 3' or equivalent is to be placed
over the leachfield.
7. The area over the trench is to be finish graded to prevent ponding of surface water runoff.
8. The septic tank and leachfield must not be closer than 100' to any existing private well, 150' to any
Class "C" well, or 200 feet to any community well.
RECOMMENDED LEACHFIELD DIMENSIONS:
TOTAL DEPTH = 4' GRAVEL DEPTH = 2' under pipe, 2" over pipe

The Right -of -Way Division has reviewed a request for a letter of nonobjection to an existing septic
system pipe, which encroaches S' Into the 10' Utility Easement on the eastern property boundary. On the
as -built survey dated July 29, 2005, submitted with the request, the petitioner has shown the existing
septic pipe.
This letter of non -objection is issued with stipulations, and by using it the petitioner Is agreeing to the
following:
1- Municipality of Anchorage (MOA) wilf be held harmless, now and forever, fur any damages or
Injury to any personas a result of the encroachments.
2. All applicable codes and regulations will be observed and maintained within the easement.
3. This letter ofnon•objection will in no way preclude MOA from full use and enjoyment of its rights
Within any portion of the easement.
4. Addiliot,al and extraordinary costs incurred during any fiuure required construction, repair or
reconstruction of MOA Improvements io accomntodate any or all of the encroachments shall be paid

Letter of Non -Objection Instructions
To properly process your request we need certain information to evaluate your application. Please
provide the following documentation or annotate the documents with the additional information:
1. One copy of the application with all the information filled out.
2. One copy of the most recent property as -built survey.
3. Show location of the encroachment (fence, wall, building, shed, eta) on the as survey.
4. Show the dimensions from the object to the property line and/or how far it projects into the
casement. Also show the height above ground.
5. After notification that we are annrovine Your request, contact each utility company that
services your arca- Provide them a copy of your plans/as-builts so they may prepare a letter of non-
objection to your encroachment(s). You are required to obtain the letters from the utility companies
even though you may not think they have facilities on your property. Each company evaluates their
own past, present and future requirements for your area. Do not obtain letters of non -objection until
you receive notification from the Right of Way Division that your application for the Letter of Non-
objection has been approved.
6. Please provide us one copy of the letters of non -objection from the utility companies.

ENCROACHMENT NON -OBJECTION AND AGREEMENT
THIS AGREEMENT, made and entered into this day of , 20
by and between MATANUSKA ELECTRIC ASSOCIATION, INC., an Alaskan non-profit
member owned electric cooperative, whose post office address is Box 2929, Palmer,
Alaska, 99645, hereinafter called "MEA", and Claudia C. and Daniel Montgomery
whose address is P.O. Box 672331 Chugiak AK 99567 hereinafter called "ADDlicant".
Property Description
Lot 2, Steele Estates Subdivision, according to Plat 87-31, Anchorage Recording
District.
Encroachment(s) Description
Portions of a septic system are located within a 10 foot wide utility easement
located along the east edge of the above referenced lot.
NOW, THEREFORE, for and in consideration of their mutual agreements MEA,
for itself, its successors and assigns, waives its objections to Applicant's
encroachments as described in the paragraphs above, subject to the following
conditions:
1. Applicant shall indemnify and hold MEA harmless from liability for any injury
or damage to any person or property which may result from said
encroachments.
2. The Applicant fully understands and agrees that this encroachment
agreement does not restrict or limit the current or future ability of MEA to
fully enjoy the benefits of the easement for any and all utility purposes that it
presently enjoys under the easement.
3. Applicant stipulates and agrees that it shall hold MEA harmless from liability
for any and all damages to the encroachments that may result from the
existing and future use of said easement by MEA, its contractors,
successors, agents, licensees, or assigns.
4. Any facilities or property of MEA that are damaged or destroyed as a result
of such encroachment(s) shall be repaired or replaced at Applicant's
expense.
5. Applicant shall comply with all applicable safety codes and regulations.
6. Any expenses which MEA may incur in the relocation or modification of its
facilities to accommodate the encroachment(s) shall be paid entirely by
Applicant.
7. MEA shall be given reasonable advanced notice should Applicant desire to
add new structures or appurtenances or modify existing structures or
appurtenances that are or will be located within the easement.

By its execution of this Agreement, Applicant agrees:
(a) for itself, its successors and assigns, that its encroachment upon
the named easement as hereinabove mentioned will be subject to the
foregoing conditions, and
(b) Applicant shall within 60 days of the date first listed above, record
this document in the proper recording district and shall provide MEA
notice of such recording by providing MEA with a certified copy of the
recorded document. Should Applicant fail to comply with this recording
and notice requirement, this agreement shall become null and void.

executed by their duly authorized representatives and agents as of the day and year
first above mentioned.
