Subject: New sewer & well permit - Sue Tawn Estates Lot 2 Add. No.1
Gentlemen:
The owner has requested we proceed forward to obtain a well and septic permit on
the subject lot On May 25, 2000 we dug one testhole for the proposed system The
results of the test is attached The general slope of this lot is from west to east at a
grade of approximately 5-10% for the western one-third of the property. From there
the slope continues in excess of 25% to Little Peter's Creek on the eastern portion of
the property.
We have designed our system utlhzmg the testhole we excavated for the 3-bedroom
house, which is proposed for this lot. The lot will be served by an individual well.
We propose to install a 2' wide deep trench Water was not encountered during the
excavation or monitoring
There are no public or private wells within 200' of our proposed system location
except as noted There is no surface water within 100' of the proposed system and
there are no known curtain drams within 50'. We do not expect there to be any
adverse effect on adjacent lots by the development of this system.

This letter replaces the one dated 9/12/85. Caulder couplings
could not be placed on the septic tank connections as these con-
nections were made by cementing the cast iron pipe into the concrete
septic tank. The connections were in excellent condition and
there is no danger of seepage. Please keep the enclosed photos
for your files.

This Department hereby reinstates a previous State DEC waiver to
87 feet for the separation distance between the existing septic
tank and well on the subject lot. This waiver is valid for a three
bedroom single family dwelling.
Changes to the existing system will require that all sewer and water
components be brought into compliance with current State and
Municipal law.

Dear Sir:
You are hereby notified that the waiver of 87 fto horizontal separation
between the well and septic tank for subject property is revoked.
Upon receiving information for the )~nicipality Of Anchorage it was
found that there was separation distance of only 50 ft. from the
septic system to a creek on the East side of the ~bJect property.
It is required by today's regulations that this distance to ~ I00 ft.
In waiver request submittals, State regulations specify that waiver
request for all ~aters subject to contamination rx~st be clearly
identified as to distance and size of that ~ter body. In your
report you failed to identify this water body and so therefore, your
waiver request has been revoked.

Dear Susan;
I have performed a health authority inspection on the above
referenced lot and have encountered several separation distance
that should be brought to your attention. These distances all
have a legitiment reason for existing, ie. priority dates or
waivers, and I have listed them for your review.
1. septic tank and leach field separation to creek less
than 100'. (installed prior to State regulation 10/73.)-~
2. well to septic tank separation distance 87 feet. waiver
obtained 7/12/85.)f
3. septic leach field directly adjacent to property line.
(approved by Municipality in that location 7/73.)

Dear Mr. Lewis:
In previous excavation to install a monitor tube in the
above referenced leach field it was found that the system was
installed using cast iron pipe with No Hub, hose clamp type coupl-
ings. The connections were found to be' in good condition and
it is assumed that this is the case at the inlet of the tank
which was not uncovered. From my inspection of the leach area-
I am reasonably sure that the tank area would have the same type
construction and ask that you approve this waivier.

Dear Mr. Butera:
The Department has reviewed the subject waiver request and hereby con-
ditionally waives the horizontal separation between the well and septic
system to 87 feet on the subject property for a 3 bedroom siu~le
family residence only.
Waiver is conditionally ~approved upon the receipt~by the engineer
giving assistance tha~ the present septic ta~k is water tight and has
been equiped with:Water tight couplings on it's inl~.and outlet con-
