Narrative & Permit Application
Dear Mr. Roth:
Eagle River Engineering Services (ERES) was contracted to design a septic system upgrade at
the above referenced property. The property's current leachfield is surcharged, and requires
immediate upgrading. We propose to utilize the existing septic tank and lift station, after a
thorough inspection. If inspection of the tank and lift station reveals any deficiencies, a new
septic tank and lift station shall be installed.
During on-line records search for this property, it was noticed that another Engineer had
submitted a design for a replacement leachfield, and had paid for a septic permit. The
homeowner informed me that they no longer wish to deal with the other engineer or utilize his
design documents. Please consider this a design change. We are assuming the homeowner will
not have to pay another septic permit fee. If you require another septic permit fee to be
submitted, please contact us and we will collect the fee from our client so we can continue the
review process. We wish to get this permit as soon as possible due to the surcharged ni ture of
the existing leachfield.
We believe the proposed 4 bedroom septic system upgrade will have very limited impact on
adjacent properties for the following reasons:
1. The surrounding lots are connected to Public water, so this lot and neighboring
lots have room for septic systems and alternate sites. 1
2. Immediate neighboring septic systems are all +20' distance, and no private (or
community) wells within 200' of the proposed septic system.
3. This permit is for a septic system only.
4. Drainage will not be affected and is not a major consideration in our design.
5. Slopes are less than 25% in the area of, and adjacent to the proposed septic site.

the septic systems or reserve areas on adjacent lots.

SPECIFICATIONS FOR ON-SITE SEPTIC SYSTEM — MOA CERTIFIED INSTALLER

A. GENERAL
1. The proposed well and septic plan is for a 4 bedroom single family residence only.
2. The drawing and or site plan shall be a part of this specification.
3. All materials and workmanship shall meet the Anchorage Department of Health and State Department
of Environmental Conservation requirements.
4. All soil tests are advisory to the design and are to be verified or modified in the field by the eng neer.
5. All excavations and depths are advisory and are to be verified in the field by the contractor to meet
Municipality of Anchorage, Department of Environmental Conservation requirements.
6. It is the responsibility of the owner to obtain all necessary permits or easements and to locate any
adjacent multi -family wells. A licensed surveyor should locate any utility easements and ex ict 100'
radius from the existing and neighboring on-site wells.
7. The excavation is to be exactly in the area shown on the site plan, any deviation requires engineer
approval.
8. Any remaining open test hole excavations shall be filled and monitor tube removed.
B. SEPTIC TANK WITH LIFT STATION
1. The septic tank shall be an Anchorage Tank 1,500 gallon septic tank with integral OSI lift station
pump model OSI -05-20-11111', or other engineer approved configuration (example: 1250 gal septic
tank with auxiliary lift basin of 250 gal min.)
2. A receipt from a licensed electrician shall be provided to the engineer verifying lift station
wiring to all applicable codes.
C. DRAINFIELD I
1. The drainfield is to follow the natural land contour to maintain uniform total depth of the di infield
bottom.
2. The bottom of the drainfield shall be level, plus or minus 1.5".
3. The elevation of the bottom of the gravel layer is not to be more than 4.5' below grade as meisured at
the monitor tube for test hole #I in the uppermost trench
4. The manifold and effluent lines are to be 1" PVC (white pipe) with 1/8" drilled holes at 2.5' on center,
holes down, within the leachfield. Line from tank to field is to be 1-1/4" HDPE (black pipe).
Transition to 1" shall be made with a king nipple.
5. The drainfield gravel is to be covered with typar fabric material.
6. Soil or combination of soil and extruded board insulation to a depth of 3' or equivalent is to i placed
over the drainfield. Mounded side slopes not to exceed 3:1.
7. The septic tank and leachfield must not be closer than 100' to any existing private well, 150' to any
Class "C" well, or 200 feet to any community well.

Septic Field Replacement Permit
EMERGENCY UPGRADE REQUEST
Ladies and Gentlemen:
My firm was contacted in regards to a failed septic field at the above referenced lot. The
owner has indicated the field is completely failed and sewage is spilling onto the ground.
There is an emanate health threat. The tank is being pumped on a daily basis to prevent
back up and spillage of sewage. I am writing to request an immediate issuance of a
septic field replacement permit for this lot. The proposed systems will serve an existing
four-bedroom house. The drain field that is in failure, installed in 1989, will be left in
place and reused. A diverter valve will be installed between the two fields inside the lift
station manhole. The existing 1500 -gallon STEP tank will be inspected and replaced if
necessary. The lot is served by the AWWU water system.
1. Soils. See the attached soils log. There has not been a soils test on this lot. I am
providing recent soils information from the surrounding lots. My firm replaced the
system across the street last year. I am basing my design on a ground water level at a
depth of 9 feet below ground surface. Bedrock was not encountered in any of the test
hole. We will erform a soils test before installin the system to veEK soil condition and
percolation rate It is my opinion that the overall soils appearance of the soils, an
application rate of 0.8 gallons/day/square feet should be used using standard treatment
system.

3. Surface Water: There is no surface water within 100 feet of the proposed system.
The proposed systems will maintain at least 100 feet from all surface water and drainage
ditches.
4. Topography: The average topography in the area of the proposed septic system is
approximately five to 10percent in the area of the SAS.
The proposed installation will not affect the future development of the surrounding or
existing lots. There are no wells or septic systems within 200 feet of the proposed septic
location. The proposed system will be located greater than 10 feet from the water service
line and water main.

SPECIFICATIONS FOR ON-SITE SEPTIC SYSTEM
LEGAL: LOT 5A, EAGLE RIVER VALLEY RANCHETTES
A. GENERAL
1. The well and septic plan are for a single family residence only.
2. The drawing and or site plan shall be a part of this specification.
3. All materials and workmanship shall meet the Anchorage Department
of Health and State Department Of Environmental Conservation require-
ments.
4. All soil tests are advisory to the design and are to be verified or
modified in the field by the engineer.
5. All excavations and depths are advisory and are to be verified or
modified in the field by the contractor to meet Municipality of
Anchorage, Department of Environmental Conservation requirements.
6. The excavation is to be exactly in the area shown on the site plan,
any deviation requires engineer approval.
7. It is always recommended that a surveyor locate the nearest lot line
position and the location of any easements.
B. DRAINFIELD
1. The drainfield is to follow the natural land contour to maintain
uniform total depth of the trench bottom.
2. The bottom of the drainfield shall be level, plus or minus 1.5".
3. The total depth of the drainfield excavation is not to exceed 5' at
any point.
4. The sewer line is to replace the exisitng sewer line that leads to
the existing tank.
5. Soil or combination of soil and extruded board insulation to.a depth
of 4' or equivalent is to be placed over the drainfield.
6. The area over the drainfield is to be finish graded to prevent
ponding of surface water runoff.
7. The septic tank and leachfield must not be closer than 100' to any
existing private well, 150' to any Class "C" well, or 200 feet to any
community well.
