Dear Veterans Administration:
An On -Site Water/Wastewater Permit, number S W000420, issued by this office for a single-family
system, will expire on October 04, 2001. This permit was valid for 365 calendar days.
If this was a well permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be sent to
this office for review, approval and documentation. This as -built inspection report must be signed by
the licensed Professional Engineer who inspected the installation of the system. As -built inspection
reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A new permit may be issued free of
charge for a second year if the application for the renewal is received on or before the date of expiration
of the original permit.
When applying for anew permit, the fees are: $320.00 for a wastewater permit and $120.00 for a well
permit.

SITE NARRATIVE; LOT 1, OLLIE WALKER SUBDIVISION NO.2 & BLM LOT 174, Sec 30,
T15N, RIW, SM is bounded by LOT 2, OLLIE WALKER SUBDIVISION NO.2 & BLM LOT_ 173,
Sec 30, T15N, RI W, SM to the Northeast, LOT 9, OLLIE WALKER SUBDIVISION NO. 2 &
BLM LOT 175, Sec 30, TISN, R1W, SM to the Southwest, the Old Glenn Highway to the
Northwest, and Tract A to the Southeast. There are homes on Lot 2 & 9, 011ie Walker Subdivision _
No. 2 with on-site water and wastewater systems. The system on the subject lot was permitted and
recorded by the Municipality in 1981.
An existing 3 -bedroom home is present on the lot. The existing wastewater system was installed in
1981. The existing system appears to be in a state of failure. It appears an undocumented wastewater
system was installed on the lot sometime between 1981 and present. This system also appears to be
in a state of failure.
The dwelling is on BLM Lot 175 and appears to be partially in the 100 -foot road easement to the Old
Glenn Highway. The dwelling is also partially within a communications easement to the rear of the
lot. The existing on-site wastewater system is also on the lot and appears to be partially within the
100 -foot road easement to the Old Glenn Highway. The existing well is on Lot 1, 011ie Walker No.
2.
The BLM lot is fairly flat and slopes from the northeast to the southwest with a slope of
approximately 2 %. The 011ie Walker lot is flat with an approximate 2 % slope to the southwest on
the northwest portion of the lot. The middle portion of the lot has a sharp 4 foot drop at the extreme
of the first third of the lot and a 100 foot drop down to Fire Creek on Tract A. This makes the lot
extremely difficult to layout. These extreme slopes are indicated on the plan view. The two lots are
approximately 112 feet to 104 feet to 67 feet wide with decreasing width from Northwest to
Southeast.
There is not sufficient room in the area between the Old Glenn Highway easement and the
communications easement to construct an on-site wastewater system on BLM Lot 174. The soils
found in this area appear to be different that those indicated on the 1981 soils logs and do not appear
to have suitable soils for such a system. Two existing failed systems seem to confirm this premise.
Due to the physical condition of the two lots and the existence of the various easements, a drastic
change in the location of the well and wastewater system was required.

SOILS TESTS: A total of three test holes were accomplished. Test hole # 1 resulted in the discovery
of an undocumented failed soils absorption system. The system was underlain with a very silty
material and no percolation test was performed. The layers in test holes # 2 and 3 are fairly consistent
and fiat with no slope. The area near the existing well could be used for a soils absorption system.
However, test hole 43 appeared to be the best location, since the percolation rate in the upper
portion of the test hole was better and room exists for a replacement system. The soils logs are
attached.
WELLS: No impact to the on-site water system on the adjacent lots is anticipated from this system.
The existing well on Lot 1, 011ie Walker must be abandoned and a new well installed in the area to
the extreme Northeast of BLM Lot 174, between the Old Glenn Highway easement and the
communication easement. Since the new septic tank will be installed less than 100 feet from the
existing well, the existing well must be filled with clean sand, concrete, or grout to above the static
water table, then the casing must be perforated and pressure grouted. After pressure grouting, the
casing must be filled with clean sand, concrete, or grout to a level below the pitless adapter. The
existing septic tank on BLM Lot 174 must be removed prior to the installation of the well.
WASTEWATER SYSTEMS: No impact to wastewater systems on adjacent lots is anticipated
from this system.
RESERVED SPACE: Sufficient room on the northeast side of test hole # 3 exists to place a
replacement system.
DRAINAGE: No impact on the drainage of the lot is anticipated from the proposed wastewater
system.
WASTEWATER SYSTEM DESIGN
Percolation Rate: Test Hole #3 = 5.38 minrn = 6 minrn.
Per Table 1, A 9048(S-1) Page 16, Loading Rate = 0.5 g.p.d./sq. ft = 300 sq. ft/bedroom.
Suggested System = Bed Drain field, 3 bedrooms x 300 sq. ft/bedroom = 900 sq. ft.
System to be installed in the strata which was percolated. Bottom of the system will be 6 inches
into the strata, which will be over 6 feet from the bottom of the hole.
Depth of gravel = 6 inches, bed width = 15 feet, bed length = 60 feet.
15 ft x 60 ft = 900 sq. ft
Suggest care be taken during installation to ensure the bottom of the bed is not smeared if the soil
is wet. The soil is silty and smearing could contribute significantly to premature failure.

Per our conversation, an additional water monitor was accomplished on the subject

All measurements are below existing ground surface and include a reduction for
monitor tube stickup.
Based upon this information, it appears the ground water level has risen considerably.
The on-site wastewater disposal system designed, submitted, and subsequently
permitted by your office must be modified to reflect the ground water table which
existed October 15, 2000. Attached, are two drawings. The plan view has been
changed to delete the extraneous existing well near the shed to the rear of the
property. The detail drawing has been changed to reflect a depth of the ground water
table to be 2.2 feet below the surface. The bed has been raised to meet the minimum
separation distance of 4 feet between the bottom of the system and the ground water
table. This makes the system a mound system. All other portions of the system remain
the same. Please replace the existing drawings in your permit file.

Minimum distance between a well and any on-site sewage disposal system is 100 feet
for a private well or 150 to 200 feet from a public well depending upon the type
of public well. Minimum distance from a private well to a private sewer line
is 25 feet and to a community sewer line is 75 feet. Well logs are required
and must: be returned to this department within 30 days of the well completion.
Other requirements may apply. Specifications and construction diagrams are
available to insure proper installation.
