Dear Mr. Shafer:
This letter is in reference to the materials received in this office December 23, 1993,
requesting final operation approval for the above project. The Department has completed
its review of the materials received which included a revised as-built of the on-site
wastewater disposal system upgrade. Based upon this review it appears that the system
was installed in accordance with the previously approved plans. Therefore, in accordance
with State Wastewater Disposal Regulations, final operation approval for this on-site
wastewater disposal system is granted. Attached is an Approval of On-Site Residential
Water and Sewer Systems verifying this Department's approval.

Dear Mr. Shafer:
This letter is in reference to the materials received in this office December 6, 1993,
requesting operation approval for the above project. The Department has completed its
review of the submitted application and as-built of the on-site wastewater disposal system
upgrade, and it appears that the system was installed in accordance with the previously
approved plans. However, based upon a review of the submitted as-built, the Department
has the following comments before issuance of operation approval:
1. A profile view of the septic tank indicates a new 1500 gallon tank. The plan
view illustrates a 2500 gallon tank which was approved for this project. A
re-submitted as-built correcting the information is requested.
2. Monitoring tubes were to be installed at the ends of the trench per this
Department's approval letter date August 30, 1993, in accordance with
ADEC guidelines. The as-built indicates placement of the monitoring tubes
near the middle of the trenches. Since the monitoring tubes appear to be
properly installed, the Department will accept their present placement for
this system.
Final approval to operate will be granted upon receipt of a new as-built to satisfy the
above comment number one. Thank you for your cooperation with this Department, if

Dear Mr. Shafer:
This letter is in reference to the materials received in this office August 10, 1993, which
included a cover letter, lot diagram, and percolation soil test results. Based on a review
of the materials submitted the Department has the following comments regarding the
approval of the on-site wastewater facilities:
Wastewater Disposal System
From the information contained in the soils log and your assumption of the in-situ soil
conditions, it appears that the on-site wastewater disposal system is adequately sized
(1500 square feet). Therefore, for the concerns of this Department, approval for the
construction of the on-site wastewater disposal system is granted in accordance with
State Wastewater Disposal Regulations (18 AAC 72) subject to the following conditions:
1. Installation of the wastewater system will need to be monitored by a
registered professional engineer (P.E.).
2. Complete as-built drawings will need to be submitted to this office under the
stamp of a P.E. The drawings will need to verify separation distances
between the water table and nearest impermeable strata.
3. Effective depth of a trench should be measured from the bottom of the
distribution pipe. There were no plan drawings submitted to indicate what
was considered the effective depth of the trench.
