Per the request of the builder (Josh Gifford), he would like the permit re -issued. A site visit was made by GEG on 5/31/2023 to confirm the site conditions and groundwater monitoring. During our site visit, the groundwater was noted as 7.25' below grade. It is important to note that there is a significant depression around testhole which may be affecting the groundwater readings. It is not uncommon for a testhole that has had surface water runoff into the monitoring tube to become clogged with silt. This leads to inaccurate groundwater readings. It is also important to note that groundwater readings were taken on following dates and found to be significantly different from the 2023 reading: • October 14th, 2003 — 13.75 feet • June 28th, 2019 —13.5 feet We are requesting that the existing permit be re -issued "as is" with the following special provisions: • Either the groundwater reading in the testhole subsides to meet MOA onsite code requirements prior to the, installation of the trench or • If the groundwater readings do not subside, a new testhole is to be installed to accurately represent groundwater. • If the new groundwater monitoring tube shows elevated groundwater, the system will be re -designed. In summary, there appears to be other factors affecting the groundwater reading. We request that the department re- issue the permit "as is" with the previously mentioned special provisions. We are proposing to install a new two-compartment IM -1060 Infiltrator septic tank and a 5 -wide trench type drainfield. One testhole was excavated on the property and the proposed drainfield and its reserve site our designed around the 30 -foot radius of this testhole. Comments regarding the design are summarized as follows: SOILS: Attached is a soil log from 2003 which shows the soils classification, percolation test data and 7 -day groundwater monitoring. The percolation test was performed between 4.0 to 5.0 feet and the absorption rate was <1 minute/inch. We are proposing to use a 1.0 gpd/sq.ft. application rate for this design with a 5 -wide trench type drainfield. Groundwater was checked on 6/28/19 and found to be at 13.5' below ground surface. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications. SURFACE WATERS: There are no surface water concerns. TOPOGRAPHY: The topography runs northeast to southwest at an approximate 10% slope. In short, there are no topography concerns. As states on the letter for the permit issuance in 2023, a new testhole was to be excavated adjacent to the existing hole to re -confirm groundwater (it was believed that the 2003 testhole was giving inaccurate groundwater readings). The contractor only had equipment to excavate to 10', however, the existing testhole had already confirmed adequate separation to impermeable and/or bedrock to a depth of 16 feet. In summary, a new groundwater monitoring tube was excavation and installed on 10/24/2023 and was dry to 10 feet deep on 10/31/2023 (confirming our assumption of the inaccuracy on the 2003 testhole). In addition, to the re -excavation of the testhole, a sieve analysis was taken on the soils and submitted to a certified lab. Per the attached sieve, the soil was classified as a SW -SM. Therefore, a sand filter is no longer required. Attached are revised drawings for a drainfield that no longer requires a sand filter. As a result, the drainfield has been reduced in length and more effective depth has been added. In addition, a new plot plan was completed, and the new drawing shows the proposed home and well location (both are currently under constructionfinstallation).
